Analyses of 29.651 hospital deaths and medical examiner cases revealed that only 3.8% to 7% of cadavers were acceptable as potential skin donors. A consent rate of 32% yielded an actual skin donation rate of 1% to 2% of deaths. A survey of 20 skin procurement services and skin banks found that 868 donors (568 m2or 6.076 ft2of skin) were obtained during 1979. Skin donation rate was unrelated to size of population base, but was correlated with the number of skin procurement personnel (r = 0.91) and was substantially increased by medical examiner sources, which yielded one half of all donors in 1979. The cost of skin was inversely related to the number of donors procured (r = −0.76), and increased cost efficiency was attained by larger-volume skin banks. These findings emphasized the potential usefulness of large regional banks capable of cryopreserved skin storage and rapid distribution upon demand.

A dramatic decrease in the concentration of cholesteryl ester in the plasma of severely burned patients accounts for the decrease in total plasma cholesterol and plasma linoleate composition. There is a significant reduction in the cholesteryl ester concentration in both low density and high density lipoproteins. The plasma concentration of apolipoprotein B (LDL) is lower than in controls 10 to 12 days postburn, but it increases to control concentration by 20 days postburn. In contrast, a more significant and sustained decrease in the plasma concentration of apolipoprotein A-1 (HDL) is observed to 70 days postburn. The burn wound apparently is a significant route of loss, since both lipoprotein particles and apolipoprotein A-1 are observed in burn wound fluid through the first three weeks postinjury. The continued low concentrations of HDL and apolipoprotein A-1 (HDL) even after wound closure remain unexplained.

A retrospective review of the charts of 52 adult male burn patients showed that a self-administered mixture of 50% nitrous oxide and 50% oxygen (Dolonox) provided pain relief without side effects in 49 patients undergoing hydrotherapy and debridement procedures. A discontinued prospective study, in which compressed air was used as a placebo, also suggested the effectiveness of the nitrous oxide-oxygen mixture when used in conjunction with narcotics for pain relief during hydrotherapy.

In contrast to other culture techniques, which cannot differentiate between surface and deep invasion, a combination quantitative and histologic culture method to predict and document burn wound sepsis has been developed that is simple and inexpensive to interpret at 24 hours. In 27 patients admitted to a burn center with full-thickness burns of more than 20% total body surface area, histologic culture of burn wound eschar allowed close observation of the dynamics of bacterial growth and reduced the amount of technician time (and thereby cost) necessary to document the degree of burn wound infection. Although quantitative cultures gave several false positive results, there were no false negatives.

Mechanical pressure used on healed second- and third-degree burns has long been accepted as a successful means of controlling hypertrophic scarring. Custom-fitted elastic pressure garments, when used alone, are not effective on some areas of anatomical contour and require that additional materials be inserted under the garment.A variety of insert materials are available, including Polycushion, Betapile, thermoplastics, and silicone elastomer. The criteria for use, fabrication method, and advantages and disadvantages of the materials are outlined.As soon as a problem area in the palm, side of the nose, breast cleavage, axilla, or sternal notch is identified and pressure can be tolerated, an insert is added under the pressure garment. Inserts also can be used to add pressure on any area where there is a raised scar or a decrease in range of motion due to a scar band. A pressure program is a vital part of a comprehensive rehabilitation protocol, but all selected treatment methods must be used together to obtain the best results. This review describes the characteristics of currently available synthetic insert materials and outlines the most advantageous use of each.
