摘要:
External fixation devices, which have been almost dormant for over two decades, are becoming increasingly popular in the treatment of the injured patient. Because of the long-range care involved with such a device, care of the patient with an external fixation device must include not only meticulous nursing care during hospitalization, but also a detailed patient teaching plan. The total approach for this care is presented along with a brief historical development of fixateurs and the indications for use.SummaryCare of the patient with an external fixation device may appear difficult at first due to the obstacles of the device itself and the possible severity of the wound it surrounds. A well-structured teaching and care plan followed by a great deal of consistency on the part of all involved will go far in overcoming many of the problems. In fact, if the word “challenges” is substituted for the word “problems,” the first major step is taken in assuring a successful prognosis for the patient with an external fixation device.

摘要:
The transfer of muscle and its blood supply to the chronic wounds of osteomyelitis is providing a cure for many victims of the disease. The establishment of a new, rich blood supply is the cornerstone of this surgical procedure. Bone, which is relatively avascular, cannot conquer osteomyelitis, and the bone is destroyed. The new blood supply allows treatment of the disease and promotes new bone growth. Postoperatively, patients are immobile, requiring frequent assessments and preventive planning by nurses. Removing all pressure from the flap is the major principle guiding nursing care of these patients.

We offer this sample script as an aid to you if you are involved in preparing patient-teaching tools. A similar script was used for our teaching aid About Your New Knee. “This aid is a notebook with photographs that is used with a tape. The nurse uses it as a tool when teaching ‘patients prior to total knee surgery. The ”preparation of this type of aid is described and our sample script shared.Summary The steps in making this type of audio-visual aid are not difficult, but require perseverance. These aids promoted a consistent quality to the preoperative teaching on our orthopaedic unit. The patient's confidence in the staff seems to be bolstered by their use. Interviews with the patients who have used the aids have been favorable. Patients seem genuinely pleased that we would go to this effort to inform them. One of our goals for the future is to evaluate the effectiveness of these aids in a more scientific manner.

Plaster casts applied in the postoperative period may become stained with blood or other fluids. The spotting process can occur over the course of several days. This can be attributed to the physical characteristics of the plaster of Paris and does not necessarily imply any condition change of the underlying wound.

摘要:
All nurses encounter long-term back clients whether they work in hospitals, offices, or clinic areas. This article will acquaint the nurse with the findings of a six-month study of the multidisciplinary approach to long-term Federal Workers' Compensation low back clients. Medical and nursing literary search failed to reveal any previous literature on this approach to Federal compensation clients.SummaryThe gross money savings in this to the government has been projected by the United States Department of Labor at $320,000 per year for these 43 clients, providing. All the variables are met – meaning the employing agency rehires these clients with the restrictions recommended.Currently our panel does not have access to numbers of claimants able to make the transition back into the, work force or numbers who are receiving medical or psychiatric treatment. The panel functions totally as a second opinion panel and does not initiate treatment.Before the evaluation, these clients considered themselves to be totally disabled: living the life and role of Invalids, and not being functional in their careers and daily activities. An exciting nursing implication is that now, after the evaluation of the multidisciplinary panel, nurses can help clients take steps towards becoming more productive people in our society.
