Cotrel-Dubousset instrumentation was designed and introduced by Drs. Yves Cotrel and Jean Dubousset. Introduction in the United States (1984) was by Drs. Kenton Leatherman, Richard Holt, and John Johnson of Louisville, Kentucky, and Dr. Harry Shufflebarger of Miami, Florida. It is a form of bilateral segmental fixation relying on the use of multiple hooks that can be used with selective lordosing and kyphosing forces and at times providing derotation by rotation of the pre-bent rod according to the flexibility of the curve. It has an application to virtually all forms of spinal deformity surgery, and its applications and acceptance are rapidly evolving.

Scoliosis is a bony deformity of the spine that affects the adolescent's body image at a very vulnerable time in life. Advanced technology, refined surgical technique and quality nursing care are health care modalities that can be tuned into the adolescent's needs. Through a team approach, new answers to progressive scoliosis are being discovered.The Philadelphia Unit of Shriners Hospital began performing posterior spinal fusions with Cotrel-Dubousset Instrumentation in August 1985. The nursing staff was challenged to anticipate needs of these patients since the literature is so limited in this area. To document this challenge, a descriptive research study was conducted to evaluate the adolescent's tolerance to the surgery and the nursing care required to promote a healthy, earlier discharge. Our results indicate that the adolescent who receives this surgery is able to return to normal growth and development tasks at a quicker pace than ever before. Nurses, as the coordinators of their care, are essential to achieving this goal.

The three major nerves supplying innervation to the hand are very susceptible to damage, which may result in severe loss of function. Even following successful nerve repair and complete regeneration the nerve, damage to soft tissues, tendons, and joints, in addition to the lack of normal sensibility that may result from the injury, can lead to a nonfunctional hand.Therapeutic intervention at the earliest possible time may prevent or decrease soft tissue damage as well as teach the patient compensatory skills that lessen functional impairment of the hand.
