摘要:
As the average age of the population increases, more elderly individuals with disabling joint diseases may require total joint replacements to improve their quality of life. The development of a postoperative wound infection could lead to extended and costly hospitalizations and perhaps even result in permanent disability. Perioperative nurses play a significant role in the prevention of sepsis during total joint replacement surgery. Perioperative nurses can effectively contribute to positive outcomes for recipients of total joint arthroplasties by keeping up with advancing technology as well as increasing their understanding of the infectious process, the proper care and handling of prosthetic implants, and the need for strict aseptic technique.

摘要:
Case management is a patient care delivery system that is transforming health care in the 1990s. The key to its success is its primary tenet of collaboration among all disciplines. Case management enhances communication among all disciplines and coordinates patient care services. Multidisciplinary action plans have been developed as a tool for the multidisciplinary team, to guide health care practitioners toward the highest quality, cost-effective care prossible. At Beth Israel Medical Center in New York City, the Multidisciplinary Care Model has proven successful on both the Orthopaedic and Rehabilitation services.

While the thought of a research utilization project may inspire intimidation and anxiety in many nurses, applying the findings of research studies to clinical practice can be an exciting way to invigorate patient care. This article describes ways of getting started in research utilization. Suggestions are included for discovering resources, as well as some practical tips for taking the first steps in what can be a satisfying way to implement cost-effective, high quality care in an era of health care reform.

Diarrhea is a common complication of antibiotic therapy and can range from mild soiling of a cast to severe and life-threatening pseudomembranous colitis. Although clindamycin is the most notorious, almost all antibiotics, particularly penicillins and cephalosporins, may also be responsible (Bartlett, 1992; Kelly, Pothoulakis, & LaMont, 1994). Because of the frequent use of these antibiotics in orthopaedic patients, antibiotic-associated enteric disease is a common problem in this population. About 15% to 25% of cases of antibiotic-associated diarrhea are caused by Clostridium difficile (Bartlett, 1992; George, 1984; Kelly et al, 1994). The majority of patients with antibiotic-associated diarrhea have no identifiable etiologic agent. Salmonella, enterotoxin-producing Clostridium perfringens (Borriello et al., 1984) and Candida albicans (Danna et al., 1991) have rarely been identified as causative agents. This article describes the role of C. difficile as an enteric pathogen and its spectrum of clinical disease, including diagnosis, treatment, and prevention of nosocomial transmission.

摘要:
A variant of systemic sclerosis (scleroderma), CREST syndrome affects a relatively small number of patients. CREST (also called limited cutaneous scleroderma) has a more favorable prognosis than the diffuse form of scleroderma. However, as with most of the connective tissue diseases, the daily issues faced by patients are frequently difficult to manage and require diligent support from the health care team. This article summarizes the clinical manifestations and potential management plan to help the patient cope with this little-understood syndrome. The nursing care model follows a problem-oriented approach for nursing management.
