摘要:
The classic critique of research is applied to a systematic evaluation of the components of research reports and company literature related to pressure ulcer products and devices. Emphasis is on the need to control variables that influence the findings; some of these are: relief of pressure, nutrition and hydration, comparability of subjects, the Hawthorne Effect and the Pygmalion Effect. Replication of research before adopting products/devices for protocol use is advocated.If the study seems adequate in all the above mentioned categories, the decision for implementation of a new product or device in clinical practice still remains. There may be factors within the institution itself which influence the adoption of new technology. Some of these may be resistance to change, cost of the product/device, or structural hindrances. Critique of research reports or company literature is important before adopting products/devices for clinical use. The more knowledgeable and clinically expert the reader, the greater the possibiities for safe, effective patient care.

摘要:
An adapted version of the mattress system classification scheme described by Clark (1987:1988) has been developed and its use illustrated with the classification of four mattress systems currently used in acute geriatric wards. The classification of these mattress systems was based upon measurements of trochanteric contact pressures. The verdicts of the classification scheme were compared with the established clinical use of each mattress, with broad agreement achieved. For example, a foam mattress used to protect patients at low risk of developing pressure sores was classified as suitable for use with “low risk patients. However, several shortcomings to the classification of mattress systems solely upon trochanteric contact pressures were described in conjunction with requirements of a more comprehensive mattress system assessment.

Prediction of surgical flap survival in a dog model was evaluated by using the technique of fluorometry. The model provides a graded variation of flap ischemia with tissue necrosis occurring at the distal end. The indicator sodium fluorescein was administered via the saphenous vein at a constant infusion rate for 20 minutes. Quantitative fluorescence measurements were obtained with surface illumination of the flap at 132 sites for 15 flaps in the five animals. Wash-in slopes and wash-out clearances, both measures of tissue perfusion, were compared with the survival of flap regions observed at seven days. The sensitivity and specificity of fluorometric wash-in and washout measurements were compared in their ability to predict flap survival. The study showed sensitivities of 100% and 95% and specificities of 97% and 86%, respectively for the two methods. The wash-in procedure with constant infusion may be preferable to the wash-out technique since it requires less time to complete. In addition, it avoids bolus injections thus reducing the risk of anaphylactoid reactions.
