The authors produced a special urethral catheter with two carbon fiber electrodes to carry out electromyography of the detrusor bladder. Carbon fiber electrodes have been found to be superior to metal electrodes as surface electrodes in physiological fluid. Subjects were 20 adults; 2 cases are shown in this paper. The results of the experiment demonstrate that it is possible to carry out electromyography of the detrusor bladder in good condition with diminished noise level. This method allows a more accurate, neurophysiological evaluation of the vesical function, as compared with traditional methods.

The recent establishment of new diagnostic procedures (Doppler ultrasound, plethysmography, selective hypogastric arteriography) permitted clarification of the etiology of impotence. Based on the fact that in 30–40% of all impotent men a vasculogenic erectile dysfunction is evident, revascularization procedures could be employed. A critical survey of the diagnostic means and therapy of vasculogenic impotence is given, indicating that penile revascularization offers a valuable alternative to penile implants.

A 30-year-old female patient presented with a mucin-producing adenocarcinoma of the bladder of urachal origin. The diagnosis and histopathology, differential diagnosis, treatment and prognosis were discussed. Usually, the diagnosis is established by history, cystoscopy and biopsy. The treatment depends on the staging and grading of the tumor. It consists of a wide excision of the transverse fascia, the umbilicus, urachus and the two umbilical arteries, the anterior part of the peritoneum and the dome or the anterior wall of the bladder. In our case, radiotherapy was given because of the young age of the patient and the gravest prognosis of the tumor.

The localization of the testosterone-estradiol-binding globulin (TeBG) in prostate, liver and metastatic lymph node cells of a patient with hormone-independent prostatic carcinoma was studied by the triple-layer indirect immunoperoxidase method using anti-human TeBG antibody. The reaction with anti-human TeBG antibody appeared to be localized in the epithelial cells of the original and metastatic tissues of the prostate, and a weakly positive stain also appeared in parenchymal liver cells. The staining of the epithelial cancer cells with anti-human TeBG antibody was uneven, suggesting that the hormone-independent cancer cell population is heterogeneous as far as the localization of the TeBG is concerned.

32 patients with stress urinary incontinence were investigated by bead chain urethrocystography (UCG) and simultaneous urethrocystometry (UGM) before and after operation. 12 patients were incontinent postoperatively. Two important determinants of the operative success emerged. Firstly, an operation-induced decrease in the urethral inclination angle or urethral mobility of at least 40 % was associated with a success rate of 81 %, while in cases with a smaller reduction the success rate was only 27 %. Secondly, excessive urethral relaxation at stress, as determined by UCM, was a sign of a poor prognosis for the operative treatment of incontinence. If the index of urethral relaxation at stress remained smaller than 0.60, none of the operations were successful despite the correction of the urethrovesical anatomy. In these cases the poor intrinsic urethral properties or general laxity of the pelvic floor may have interfered with the operative results. A positive correlation existed between urethral mobility and urethral relaxation at stress. A combination of UCG with urodynamic methods seems to give additional information on the factors contributing to postoperative incontinence.

A conjugate of fluorescein isothiocyanate and bovine serum albumin bound with R 1881-3-carboxymethyloxime was used to visualize the location of androphilic proteins in tissues from patients with prostatic cancer. The fluorescence was located in the carcinoma cells but not in the stroma. Although intensity of the staining was varied in fluorescence-positive cases, almost all carcinoma cells were stained. On the contrary, positively stained cells were scarcely observed in fluorescence-negative cancer. Among 46 cases of untreated prostatic cancer examined, 76% were fluorescence-positive. 7 cases out of 9 who had relapsed from endocrine therapy showed negative fluorescence. It was concluded that the fluorescence in the tissue sections of the untreated prostatic cancer were well correlated with the histologic grade, and that fluorescence-positive cases had responded well to the endocrine therapy, as judged 6 months after the start of treatment.

The electric impedance of urine is considerably lower than that of urothelium. Thus, electrodes placed in the urethra can be used to show when urine is present. A special catheter with three sets of measuring electrodes has been employed to demonstrate the progress of the bolus of fluid entering urethra during coughing. The volume, speed and route of the bolus has been examined in a sample of results.

We devised a simple and economical technique for measuring crystallization inhibitory activities. Calcium ions and oxalate ions diffuse towards one another from two adjacent wells in a layer of gel on a microscope slide. Where they meet, a streak of calcium oxalate crystals forms. Substances which influence crystallization are caused to diffuse into this middle zone. Depending on the inhibitory potency of the substance, the calcium oxalate streak varies in density and extent. These differences are recorded photometrically by comparison with a sample containing no inhibitor, and the inhibition index is calculated. Some of the best known inhibitors were tested and showed a definite effect in this model. Magnesium and citrate in physiological concentrations are of special significance. The expected range of variation of inhibitory activities in human urine is covered by this method.

A newly devised technique for nephrolithotomy was applied in 29 patients with renal calculi, and good results were obtained. The principle of this new approach is the method of suturing the renal pelvis and the renal capsule. In this procedure paired hemostatic sutures are inserted to appose the cut surfaces and then ligated. The time for the disappearance of macroscopic and microscopic hematuria and the ischemic time needed for operation were measured and computerized tomography was performed in the 29 cases operated on by this new method. The results were compared with those obtained from 41 patients treated by conventional vertical interrupted parenchymal suture. These clinical data as well as microangiographic findings in animals have shown that this new procedure is far superior to that using vertical interrupted parenchymal sutures and is simple to use with better preservation of renal function when compared with anatrophic nephrotomy.

Pyonephrosis is caused by chronic obstruction of the upper urinary tract. Consequences of chronic obstruction are a functional defect of the mucosa of the ureter, diminished blood supply, and an increased interstitial fibrosis of the obstructed kidney and ascending infection. Penetrating through the transitional epithelium, bacteria come into the parenchyma of the kidney. In the region of vessel-connective tissue the immune response is disturbed. If the endotoxins of gram-negative bacteria come into the blood circulation before neutralization by humoral antibodies, urosepsis can develop.
