A simplified and standardized pre- and post-operative electrolyte supplement technique for primary aldosteronism was discussed from the data of balance study on 32 cases treated in our clinic since 1957. Results were summarized as follows: (1) During few days after removal of the adrenal tumor, a remarkable loss of both sodium and potassium was observed in the majority of the cases. (2) In this period, sodium supplement resulted in more sodium loss. The potassium loss was regulated under the direct influence of serum potassium level. (3) These responses might be caused by following three factors; proximal tubular rejection of sodium, disturbance of distal tubular function, and inactivation of renin-angiotensin-aldosterone system. (4) A restoration of proper electrolyte regulation mechanism was being apparent during the second postoperative week. (5) An excessive preoperative potassium conservation with sodium elimination, accordingly, might be followed by postoperative sodium deficiency. A low sodium and high potassium supplement during approximately 5 postoperative days and a high sodium administration after that period would be necessary. A program of the electrolyte supplement based on above-mentioned findings was demonstrated on table II. (6) Steroid substitution is unnecessary except that the bilateral adrenals are totally dissected. (7) An excellent prognosis including blood pressure was obtained from patients treated in our clinic.

A simplified and standardized pre- and postoperative treatment technique for Cushing’s syndrome and pheochromocytoma was discussed from experiences on 21 and 18 cases, respectively, treated in our clinic since 1956. Results were summarized as follows: Cushing’s syndrome: A subtotal adrenalectomy with pre- and postoperative irradiation to the hypophysis by a large dosis of telecobalt was effective for the cases with bilateral adrenal hyperplasia. A standard program of postoperative substitution therapy, which was composed of minimized cortisol supplement without ACTH administration, was described. Pheochromocytoma: Three methods presented for blood pressure control after the removal of pheochromocytoma, i.e., noradrenaline drip method, blood overtransfusion method, and the treatment with receptor blocking drugs, were evaluated. Results of the hemodynamic determinations in the pre- and postoperative period were demonstrated. The inter-relation of the adrenal gland and the kidney can be really demonstrated when these two glands, or one of them, show pathological alterations. In cases of nephrosclerosis, the adrenal gland shows notorious modifications in its structure, even if clinical studies through biological analysis of the secretions (blood and urine) would not show great alterations. We can find very important morphological alterations of the glomerular layer in the cortical zone and of all the medullar zone structure. The unilateral adrenalectomy improves the clinical state of the patient in many of these cases of nephrosclerosis.

In rats, the nephrocalcinosis and mortality produced by an intravenous injection of mercuric chloride were markedly diminished by hypophysectomy performed 7 days earlier. Triamcinolone in combination with hypophysectomy completely abolished the nephrotoxic and the fatal effect of mercuric chloride.

Crossed renal ectopia without fusion is a rare condition, but even more rare is its association with anomalies of the genital system. A case of crossed renal ectopia without fusion associated with agenesis of the vas deferens on the ipsilateral side is presented. Radioisotope scanning in different positions was used for the first time to verify this renal anomaly. Since scanning has proved to be as reliable as aortography, and is much less dangerous, we belive that it should be used for diagnosing this condition.

Volume and length of excised ureteral segments were measured for normal, hypo-used, and hyper-used dog ureters at increasing and decreasing pressures. Incrementally increasing pressure tended to produce substantial incremental increases in both length and volume in all subjects and use-types, although considerable differences between subjects were evident. These pressure effects were specific to the use-types, at least in the case of volume. The two non-normal use-types, hypo-use and hyper-use, were accompanied by statistically significant differential effects on length and on volume as compared to normal use-types.Hysteresis effects-specifically, tendencies for length to decrease and for volume to increase in the pressure readings during ascending pressures as compared with the readings during descending pressures-would appear to be common in all use-types. However, a statistically significant differential effect was found only in the case of hyper-use. In any case, these effects appeared to be specific to pressure level as well as use-type.

A method was initiated in which an irrigating fluid was used to inflate the urinary bladder. Over 12 patients were treated by this method. At the end of the first stage of the treatment involuntary contractions of the detrusor muscle occurred. At the end of the second stage voluntary voiding of the bladder was obtained by means of conditional reflex. Better infection control in the urinary tract resulted, and incontinence was reduced. The treatment was completed in several days.

With consideration of immunologically favorable localization, tissues from human prostatic adenomas were transplanted in the anterior ocular chamber of male guinea pigs. Not in any instance were viable implants demonstrable after 3 weeks. Similar results took place in animals treated with sex-steroids. Round cell infiltration and vascular invasion apparently testify to immunologic rejection.

Perineal puncture procured material from 10 patients with clinically and microscopically proven prostatic cancer, both before and after intensive female-hormonal therapy and after castration; the tissues were studied histochemically and microscopically. Apart from tissue acid-phosphatase, the following were investigated: succino-dehydrogenase (SDH), isocitrate-dehydrogenase (LCDH), β-hydroxybutyrate-dehydrogenase (β-BDH), glucose-6-phosphate-dehydrogenase (G-6-PDH), glutamate-dehydrogenase (GDH), lactate-dehydrogenase (LDH). Following hormonal therapy and castration there was, in both the solid cancer and in adenocarcinoma, a readily demonstrable fall of enzymatic activity compared to that at initial puncture. Striking was the transition to dedifferentiation of the adenocarcinoma following hormonal therapy and castration despite improvement in clinical palpation. In the presence of negative stromal reaction, the tumor cells of the dedifferentiated cancer there was obvious activity of the GDH, LDH, β-BDH and acid phosphatase. There was slight to no activity of citrate and pentose-phosphate cycle.
