Reproducible split renal function studies under urea diuresis have made possible the accurate identification of those patients with hypertension secondary to an occlusion of a branch of the main renal artery. Differential renal function studies are presented on 13 patients with segmental renal ischemia due to branch arterial occlusion. These cases all show evidence of excessive sodium and water reabsorption on function study; comparison of the UpAH has been shown to be more diagnostic of renal ischemia than changes in UInulin. The major difficulty in evaluating patients with hypertension occurs in those who exhibit no excessive water reabsorption on differential function study. The important observation for diagnosis in these patients is to compare the ratio of urine flow rates to the ratio of renal mass. In those patients with hypertension as a result of unilateral pyelonephritis renal function studies are of limited value.

An einem Krankengut von 47 Patienten mit postrenaler Anurie wurde das Ausmass der Entlastungsdiurese nach Beseitigung der Harnwegsobstruktion geprüft und bei einem Teil der Patienten die renalen Elektrolytverluste gemessen. Hydronephrosis and simulated intrapelvic calculi were produced in laboratory animals. Cultures of proteus and E. coli were introduced in these kidney preparations through an extremely fine (PE 10) polyethylene catheter. Massive doses of appropriate antimicrobial agents could not clear the infection and the kidney progressed to pyelonephritic atrophy. Conversely, infection could not be maintained in obstructed kidneys or if the calculus was removed before the pyelonephritic changes became irreversible.

Eine neue Anwendungsmöglichkeit der hypothermen Perfusion bei Nierenoperationen wird beschrieben. Die Methode besteht in der Einführung von Polyäthylen-kathetern in die Nierenarterie und Nierenvene und der selektiven Perfusion der Niere mit unterkühlter Spülflüssigkeit. Bei chirurgischen Eingriffen am Nierenparenchym und bei operativer Beseitigung von Nierenarterienstenosen oder Aneurysmen kann dadurch der intraoperative Blut-verlust wesentlich verringert werden und eine langdauernde Abklemmung des Nieren-stiels ohne erhöhte Anoxiegefahr durchgeführt werden. The final product of the purine metabolism differs in the various animal species; only the dalmatian dog and the primates excrete uric acid in the urine, as man does. For over a year seven female dalmatian dogs have been used in a study on different methods of lowering the uric acid excretion. Uric acid derives from the oxidation of xanthine through a reaction catalyzed by xanthine oxidase. Four possible ways of interfering with this reaction were experimented, in order to lower the uric acid which reaches the kidneys and is excreted. I. Lessen the enzyme available. Xanthine oxidase has in its molecule molybdenum, which is necessary for the enzyme activity. The administration of tungsten which issimilar to molybdenum, causes a lowered enzymatic activity. This action of tungsten, formerly described by Sorrentino in man, has been confirmed in the dalmatian dog. II. Interfere with the xanthine enzyme reaction using a great amount of an anti-metabolite of xanthine, allopurinol, which by a mass action blocks the reactive sitesof the enzymatic molecule, excluding them from contact with xanthine. By means ofsuch a mechanism, allopurinol is able to provoke a lowered uricosuria. III. Bind the uric acid as soon as it is being formed, to prevent it from entering the kidney in an eliminable form. The anti-uric acid antibody production could theoretically fulfill this goal, but in practice the immunization induced in the dalmatian dog does not interfere with the uric acid excretion. IV. Block the xanthine oxidase inducing the production of anti-enzyme antibodies. Immunization against milk xanthine oxidase causes no changes of uricosuria. In the final analysis, both tungsten and allopurinol are able to lessen the uricosuria in the dalmatian dog. Tungsten shows its action after a latency period which is longer than allopurinol, and causes a greater and longer lasting lowering of uricosuria. Allopurinol acts much faster, but its action disappears rapidly.

Macroscopic and microscopic hematuria consequent on varicosity of the venous periureteral plexus is rarely encountered. Eleven cases have so far been described in the literature some of which – showing massive hematuria – underwent urgent nephrectomy. As in the case mentioned above, diagnosis is based mainly on urographic signs since the clinic symptomatology is not clearly defined. Such signs consist of impressions on the nephric duct and pelvis, with special features. Differential diagnosis should be related to those affections that may lead to a similar radiological pattern (such as vesicular pyelo-ureteritis or impressions due to pathological lymphatic glands). Bei 20 Hunden wurden Vesiko-Vaginal-Fisteln (10 Hunde) und Vesiko-Rektal-Fisteln (10 Hunde) angelegt und bei den überlebenden 14 Tieren die Fistel 8 Wochen danach mit Hilfe von lyophilisierter menschlicher Dura verschlossen. Bis auf ein Tier war der Fistelverschluss (bis zu einem Jahr Kontrollen) permanent. Studies of urinary mucoproteins were carried out in newborn and infants of hyper-(village) and hypo-endemic (city) areas in North-east Thailand. The findings are as follows: (1) Village newborn excreted significantly higher amount of total non-dialyzable solid (TNDS) than the city group who excreted comparable to the North American newborn. (2) Village newborn and infants excreted significantly lower percentages of 1,000 to 5,000 MW and higher of the above 5,000 MW fraction. At the present time, the reason for the difference and the significance of these finding are not known.

Innerhalb von 25 Jahren wurde in unserer Einrichtung bei 354 Patienten eine Harn-leiterdarmanastomose durchgeführt. 97 Patienten konnten nachuntersucht werden. Dabei wurde das Hauptaugenmerk auf Veränderungen des Mineralhaushaltes gelegt. Na+-, K+-, Ca++-, Mg++- und CI––-Bestimmungen im Serum wurden durchgeführt und die Ursachen ihrer Abweichung diskutiert. Ausserdem wurden Alkalireserve- und Harn-stoffuntersuchungen beim gleichen Patientenkreis vorgenommen. Die pathologischen Werte unserer Untersuchungsergebnisse haben wir prozentual aufgeschlüsselt und ihre Bedeutung für die Klinik aufgezeigt. Eine Methode zur Austestung der Empfindlichkeit von Urotheltumoren auf die lokale Chemotherapie mit Thio-Tepa auf der Basis einer Beurteilung morphologischer Veränderungen in Tumorzellen im Harnsediment wird vorgestellt. Das Auftreten regressiver Veränderungen in den Tumorzellen 48 h nach der ersten Thio-Tepa-Instillation (60 mg Thio-Tepa, gelöst in 60 ml phosphatgepufferter isotoner NaCl-Lösung) weist auf eine Empfindlichkeit des Tumorgewebes hin. Zwischen dem Ausmass der degenerativen Zellveränderungen nach der 1. Instillation und dem Ergebnis der Therapie nach 12 Thio-Tepa-Instillationen konnte eine signifikante Korrelation fest-gestellt werden. Die Beurteilung der Morphologie der Tumorzellen im Harnsediment ermöglicht die Prüfung des zytostatischen Effektes auf das zu behandelnde Tumor-
