We report a case of intestinal obstruction secondary to cecal volvulus following a palliative nephrectomy. Cecal volvulus and other causes of acute abdomen should be considered in postoperative patients vwho develop adynamic ileus undergoing retroperitoneal surgery.

81 otherwise healthy men with an average age of 67 years and verified prostatic hypertrophy were randomized into two groups for either ‘minimal’ or ‘total’ transurethral prostatic resection (TUR-P). Interviews on sexual life were made preoperatively and 6 and 12 months postoperatively. 58 men (72%) were prior to the operation sexually active. 18 (31%), mainly men of advanced age, discontinued sexual activity following TUR-P. 40 (69%) remained active. 19 had retrograde ejaculation. No significant difference was found between ‘minimal’ and ‘total’ TUR-P concerning the effect on sexual activity and the occurrence of retrograde ejaculation.

Ureaplasma urealyticum is considered an etiologic agent in urogenital tract infections, especially prostatitis. Using the ‘four-specimen technique’, diagnosis can be based upon significant numbers of these microorganisms. In ejaculate, the critical number seems to be 103 cfu/ml of semen to discriminate between real infection and contamination during urethral passage. In our study, 46 of 412 samples (11.2%) exceeded this critical number. Most but not all patients suffering from ureaplasma-associated prostatitis established by the ‘four-specimen technique’ revealed significantly high ejaculate numbers, whereas all samples from patients with prostatodynia and healthy controls had lower numbers. In these cases, numbers of round cells in semen, i.e. all leukocytes and spermatides, were significantly increased as compared to prostatodynia. A significantly negative correlation was detected between the numbers of ureaplasmas and zinc concentration in semen, and an almost identically negative correlation to the content of fructose, thus indicating secretory dysfunction of the accessory glands in ureaplasmal infections of the prostate.

We report here a 70-year-old man with prostatic malacoplakia which is an extremely rare disease. This diagnosis was obtained from histological examination by prostatic biopsy because the patient was suspected having prostatic cancer on digital examination.

An animal model was established to test the effect of a calcium antagonist on nephrocalcinosis, which was induced by an atherogenous diet, and its effect on the excretion of calcium and other parameters relevant for stone formation. With the administration of nifedipine (Adalat®), the grade of nephrocalcinosis could be significantly reduced. Furthermore, with nifedipine the excretion of calcium and sodium in the urine was raised, while phosphate and potassium levels were lowered. The excretion of magnesium and citrate, reduced by an atherogenous diet, could be raised significantly with the administration of nifedipine. The pathophysiological mechanisms underlying the effect of nifedipine on nephrocalcinosis and on the excretion of the urine parameters are discussed. Apparently hypercalciuria is the result of a reduced reabsorption of calcium in the tubulus. The inhibitory effect on the genesis of nephrocalcinosis is possibly due to the lower calcium influx into the tubular cells.

During the last years several cases of acute urinary retention, due to genital herpetic infection, have been described, especially in youngsters. The condition is caused by neurogenic dysfunction due to a sacral meningomyelitis. The clinical picture before the acute attack is characterized by painful genital eruptions; fever and malaise; tender inguinal lymph nodes; paresthesia in the perineum and on the inside of the thigh, and obstipation. Skin and mucous membrane manifestations are often sparse or located so that they are easily overlooked. The treatment is bladder drainage until the patient is able to void spontaneously. Most of the patients regain normal bladder function within 10 days, but bladder dysfunction for 5 weeks has been described. We present a typical case of acute urinary retention due to genital herpetic infection.

Transrectal ultrasound of the prostate is a cheap and well-reproducible examination for the prostate and seminal vesicles. The local tumor extension of a prostatic carcinoma can be visualized. By documentation of the ultrasound image on a polaroid paper picture, the topometric section of the prostate can be obtained. This picture can be used for dosimetry in individual radiation therapy-planning. Computed tomography is performed for detection of enlarged regional lymph nodes and demonstration of the topographic anatomy of the prostate in the lower pelvis.

Between September 1977 and September 1980 we implanted 4 Rosen inflatable incontinence prostheses in 4 men with urinary incontinence. Twelve operations were necessary on these 4 patients to assure them to be continent during a total of 43 months. Because of the great number of complications we no longer use the Rosen prosthesis.

A 7 3-year-old man presented with pain in the right flank and fever. A ureteroduodenal fistula was demonstrated by intravenous pyelography and excised with nephroureterectomy. This seems to be the 8th case of such a lesion.

The combined histologic and cytologic grading of carcinomas of the prostate is not only important in evaluation of prognosis but determines the choice of therapy with knowledge of staging. Tissue biopsies of 2,200 prostatic carcinomas were classified and graded histologically and cytologically. Furthermore, autoradiographic studies were performed on 69 needle biopsies with 3H-thymidine. The cytologic parameters were correlated with cell-kinetic parameters in classification. Several subgroups of degrees of malignancy were found. Grade la corresponds to highly differentiated glandular carcinomas by histology and cytology. Grade lb carcinomas were histologically well but cytologically moderately differentiated. Grade IIa carcinomas are histologically moderately to poorly but cytologically moderately differentiated. Grade IIb carcinomas correspond to poorly differentiated tumors by histological and cytological criteria. Grade III carcinomas are in most cases undifferentiated. This differentiated grading is particularly important for therapeutic consequences in incidental carcinomas. The therapeutic consequences of grade la carcinomas are frequent controls and a wait-and-see attitude. Grade lb and IIa carcinomas must be treated according to their clinical stage by total prostatectomy. Grade lib and III carcinomas need a palliative treatment by hormones, castration, irradiation or cytostatic drugs.
