An analysis of prothrombin time (PT) standardization methods is presented. The present recommended ratio method demonstrated complex calibration and wide therapeutic ranges. Standardization by coagulation activities resulted in different therapeutic ranges due to the different sensitivity of assays for the protein induced by vitamin K absence (Pivka) inhibitor. A new method – the modified coagulation activity method – applied a reference assay against which any PT assay can be calibrated by clotting times, and the results were expressed in coagulation activities by the reference assay. The calibration of prothrombin time (PT) assays was studied. A simple calibration between pooled normal plasma and pooled stable plasma was as satisfactory as more complex calibration methods based on linear or orthogonal regression between log clotting times of abnormal and/or normal plasmas.

Forty-four patients with schistosomiasis mansoni were divided into two groups: a group of 31 patients with hepatosplenomegaly and a group of 13 patients with advanced hepatic fibrosis and ascites. Both groups were compared to a control group of 14 healthy subjects. Screening of the extrinsic and intrinsic coagulation pathways was performed by PT (%) and aPTT. Functional assays of vitamin-K-dependent coagulation proteins: factors II, VII, IX, X, and protein C were conducted by using standard methods. A progressive reduction in PT (%) and prolongation in aPTT were detected in the diseased groups. Furthermore, all studied coagulation proteins showed a progressive reduction in their activities, which became more pronounced in the group with advanced ascites. We conclude that the significant reduction in vitamin-K-dependent coagulation proteins might add to the coagulopathy and bleeding diathesis noted in patients infected with Schistosoma mansoni.

The sheep haemophilia A, recently described in a flock of White Alpine sheep, is probably caused by the production of a less potent mutant of factor VIII (F-VIII). Other potential mechanisms, such as drastic reduction (or lack) of F-VIII production, or occurrence of an F-VIII inhibitor, are less likely. This conclusion was based on the analysis of plasma dilution-clotting effect relationships in normal female (anestrous, non-pregnant) sheep, female carriers of haemophilia A, and afflicted male sheep. A response model underlying the analysis was suggested, its validity tested, and a mathematical analysis suitable for routine evaluation procedure recommended.

This study explored the relationship between cytokines (TNF, IL-1, IL-6), coagulation and fibrinolytic factors in the early stage of sepsis syndrome and the relation between these factors with the severity of inflammatory illness as measured by the Simplified Acute Physiology Score (SAPS). Twenty-one normal controls were compared to 34 patients divided into three categories ranging from uncomplicated postoperative patients, to patients with severe infectious conditions including septic shock. A major hemostatic imbalance was demonstrated with particularly marked reduction in fibrinolytic activity [drop of antithrombin III (ATIII) and protein C with an increase of plasminogen activator inhibitor (PAI-1) levels] which were directly correlated with the severity of the inflammatory state. Both ATIII and PAI-1 levels were correlated with the levels of TNF and IL-6 and the severity of illness as measured by SAPS. We established an index, ATIII/PAI-1 antigen that is significantly different among the four groups (p < 0.001) and strongly correlated with the SAPS (p < 0.001). As PAI-1 could be secreted not only by TNF activating endo-thelial cells but also by hepatocytes activated by insulinemia, treatment of sepsis with cytokine-specific agents might be of limited effect.

Increased plasma activity of plasminogen activator inhibitor 1 (PAI-1) is considered as a risk factor for thrombosis associated with atherosclerosis by reduction of fíbrinolysis. Since nephropathic patients with non-insulin-dependent diabetes mellitus (NIDDM) are a cardiovascular high-risk group, which has yielded only controversial results as to the regulation of PAI-1, we compared 19 overt nephropathic NIDDM patients (mean age 63 years, serum creatinine 1.9 mg/dl, proteinuria 4.2 g/ day) to 17 nondiabetic nephropathic patients with various causes of renal insufficiency (mean age 63 years, serum creatinine 2.8 mg/dl, proteinuria 3.9 g/day). We found normal PAI-1 levels for patients with diabetic nephropathy and significantly elevated PAI-1 levels within the upper normal range for nondiabetic nephropathic patients. Common risk factors in both groups were very high levels of fibrinogen, lipoprotein(a), serum cholesterol, and LDL cholesterol
