摘要:
HE WORLD-WIDE propaganda on painless childbirth andTwilight Sleepformed a nucleus for a greater future for motherhood than was anticipated. That the world is growing weaker and wiser is clearly demonstrated in obstetrics, childbirth becoming a more difficult problem in each generation, with the expectant mother becoming well versed through the press and magazines of the advancement of the scientific methods of pain relief to be had by merely demanding them. We no longer believe that labor pains do not hurt and the old maxim ofletting nature take its course, in the sense in which it has always been applied in obstetrics, is a thing of the past. The demand for better obstetrics and safer analgesia and anesthesia must be respected. Today nitrous oxid-oxygen is accepted as the safest method of analgesia and anesthesia in normal labor and operative obstetrics.

IN DISCUSSING this subject, which was suggested to me by your President. Dr. Isabella C. Herb, I am uncertain as to how generally local anesthesia is combined with gas-oxygen in practice. I trust that this will not be an old story to the majority of you. A fact that, however, encourages me in presenting this matter is that in seeing Cesarean sections done in several of our Eastern centers, this combined technic of anesthesia was not used.

COTTON PROCESS ETHER is a physical mixture of pure sulphuric ether, carbon dioxid and ethylene, compressed for convenience sake as well as for economy, into liquid form. The difference in diffusibility of the gases it contains is such that, upon the opening of the package to pour the liquid into an anesthetizer or to use by drip upon the usual open cone, a great deal of the gas of highest volatility is dissipated into the atmosphere and the value of the original mixture lessened in just that degree.

摘要:
ANESTHESIA, as a phenomenon of asphyxiation, has been known since the ancients. The fact, however, that men become unconscious and suffer loss of pain on sensory stimulation has' not so long been known to be dependent upon lack of oxygen alone. If one searches the older literature, all the evidences along our theme are found under the head of asphyxia and suffocation. The usual asphyxial condition, as in drowning or hanging or other methods of suppressing respiration, is not a simple case of oxygen want. The factor of carbon dioxid accumulation is always associated. The separation of these two conditions is absolutely necessary to attribute to each its proper value.

IN THE METHODS of securing general anesthesia most commonly employed, the liquid anesthetic is applied to an absorbent material so placed near the patient's face that the inspired air passing through the material becomes charged with the vapor of the anesthetic. With these methods a certain amount of rebreathing is inevitable. If rebreathing isconsiderable, the method is known as aclosed method; if rebreathing isslightit becomes anopen one.The present tendency is toward the adoption of one or the other of these methods and its indiscriminate use for all cases. Yet these methods at the best are crude, unscientific, and unworthy of consideration alongside modern rehned surgical technic. In some of the more delicate operations, such as those involving the upper air passages, the usual methods of anesthesia are a distinct handicap to the surgical work. To find satisfactory methods of anesthesia for all cases we must look among the unusual methods.

STUDY OF THE abnormal constituents of urine with reference to toxemias and the anesthetic risk has been pursued by many able investigators all over the civilized world. It is hardly possible for me to add much of value to the store of knowledge already accumulated so far as urinary pathology is concerned. My desire has been, on the other hand, to investigate the role of the physiological constituents in so far as their relation to one another may he affected by toxemias.

IT IS NOT our intention to dwell on the history of anesthesia. Only, it is worthy of mention that not long after the discovery of nitrous oxid, Humphrey Davy, the first superintendent of the Pneumatic Institute of Clifton, England, while working on the then known gases, noticed that the inhalation of nitrous oxid relieved pain. His friend Southey, the poet, used to visit the laboratory frequently and inhaled some. So impressed was he that the following eulogy came from his pen to his brother,“Oh Tom! Such a gas has Davy discovered! Oh Tom! I have had some, if makes me strong, and so happy! So gloriously happy! Oh excellent gas bag! Tom, I am sure the air of heaven must be this wonder working gas of delight.”The anesthetist of today often hears this paraphrased, especially by the parturient women. This was written in 1798 and yet it was not until forty-six years later that this elastic fluid was used as an anesthetic, and then chiefly in dentistry for another twenty-four years, until Joseph Clover, surgeon and anesthetist, adopted nitrous oxid for general use in 1868.

AT THE REQYEST of the Council on Pharmacy and Chemistry of the American Medical Association the Committee secured some samples of butyn, a new local anesthetic. which the manufacturers, the Abbott Laboratories, provided, and submitted it to animal experimentation and clinical trial. These experiments were begun several months ago by the individual members of the committee, and up to the present time seem to have been sufficiently extensive and conclusive to justify the report herewith submitted.
