Although considerable has been written on the subject1 5, the need for a better understanding of its mechanism of action as well as of the dangers involved in its administration is still apparent.

&NA;THE IMPORTANCE of a thorough knowledge of the anatomical structures encountered in nerve blocking is quite obvious. The well trained anesthetist knows exactly the nerve supply to a given area and the position and relation of the various structures through which the needle must pass to reach its proper point of destination, The dental surgeon who wishes to master the technique of nerve blocking anesthesia, therefore, must possess a thorough knowledge of the anatomy of the oral cavity. This knowledge gives him confidence, which is essential in the successful administration of conduction anesthesia.

&NA;THE METHODS of anesthesia, introduced in surgical practice scarcely a hundred years ago,* *have made great progress in regard to the nature of anesthetics and the manner of their administration. On the other hand, few researches have been made on the study of the measure of depth of anesthesia. On this specific point analgesia has been better studied and recently a new method of research of the threshold of the pain, improved by J. D. Hardy. H. G. Wolff, and H. Goodell,5has given rise to precise measures of analgesia produced by different substances.

&NA;SINCE THE INTRODUCTION of barbituric acid derivatives as sedatives a tremendous amount of literature has accumulated on the subject. Scores of such preparations have been described, and eulogized. There have been literally thousands of reports of the use of these sedatives in various conditions. Parallel to, and behind this clinical testing, has been continued research in many commercial laboratories. The latter work has been directed at preparing and proving new combinations of barbituric acid.

&NA;SINCE THE ADVENT of lumbar anesthesia as an aid to surgery, there has been a slow but steady improvement in techniques and solutions brought about by workers who were continually striving to obtain added safety, with consistent anesthesia. Earlier drugs have been replaced by newer ones, with a view to lessening toxicity in some cases, of increasing length of anesthesia in others. The trend has been toward consistent anesthesia, and, with this in view, barbotage for dilution, and fractional dosage technique have been the most important changes in technique in the past two decades. It is to these two, dilution, and fractional dosage technique, that we wish to draw particular attention, with special reference to a low dilution of procaine in normal saline, its use and technique in continuous lumbar anesthesia.

&NA;HARDLY NEW is the fact that extreme cold numbs exposed portions of the body. Any child who has made snow balls with his bare hands is familiar with the sensation of numbness of the fingers which follows the initial feeling of cold and tingling. But the use of this principle as a method of anesthesia is new, together with the rational and the physiological principles behind it. In using reduced temperatures as a means of anesthesia, we are employing an entirely different concept of anesthesia.

&NA;FOR FOUR YEARS I have used pentothal‐sodium‐oxygen anesthesia in all surgical operations, both private and clinic cases, regardless of the length of time required. In 1938, I visited a clinic in a neighboring state and saw this anesthetic being used. I realized its potentialities and immediately began its use. It was first used in Georgia at the Georgia Baptist Hospital in Atlanta, and later in other hospitals in Atlanta and now throughout the State.

&NA;WHEN YOU are faced by a problem, the solution must be made with truth and integrity, you ‐must determine the causes that prevail in order to solve it in sequence of their importance. No one will deny the grave responsibility that confronts the anesthetist in determining the best method of giving anesthesia, in order to satisfy the surgeon and protect the patient. Both the surgeon and anesthetist share the professional responsibility for the success of the operation. In fact, on many occasions, surgical success depends mostly on the type of anesthesia and the skill of the anesthetist.
