This integration requires thoughtful organizational planning. The Best Practice Health Care Map provides a model for managing the characteristics of quality clinical care within the framework of multi-disciplinary professional practice. The model incorporates the contributions of various forms of evidence into pre-specification design and point-of-care design.

摘要:
The unexpected expansion of a 14-bed medical intensive care unit exposed the need for a structured preceptor/orientation program. Staff feedback and a review of quality indicators further substantiated this finding and revealed a decrease in standards of practice and problems in staff development. A restructured preceptor/orientation program yielded improvements in communication and staff satisfaction. Preceptors and orientees now have a better understanding of their roles and expectations, and quality of care is improving in an environment that promotes personal and professional growth.

When family members became dissatisfied with a restrictive visiting policy in a combined coronary and medical intensive care unit, this situation was seen as an opportunity to better meet patient and family needs. A review of the literature indicated that open visitation policies enhance patient and family satisfaction, while a survey of patients, families, and health care team members revealed a desire for a more open visitation policy. Nursing staff, with input from other disciplines, developed and implemented a less restrictive visitation policy. Post-intervention surveys revealed higher patient and family satisfaction and a marked decrease in formal complaints.

摘要:
The continuous health disparities for African Americans when compared with Caucasians raise the question of whether health care objectives designed for African Americans actually address their needs and wants. To explore quality-of-care dimensions from the African American perspective, a combined focus group and modified Delphi approach were used. Three quality-of-care dimensions (patient, provider, and setting roles) were deduced from the data. Qualitative thematic narratives were abstracted. Because African Americans emphasized processes of care, their health outcomes may improve if process factors are judged as important as those of input and outcome in an adapted quality-of-care framework.

Risk management is an objective strategy that seeks to eliminate or decrease the chances of financial loss and legal action. Risk management focuses on an actual mistake, an accident, or a potential similar occurrence. Risks occur in nursing in a variety of practice settings. Nursing has identified several key areas in which systematic change has improved and decreased the risk factor encountered in practice. This article targets medication usage and administration and the methods introduced in quality improvement processes to decrease risks and potentially reduce financial and legal risk.

Perceptions of patients (n = 135), physicians (n = 99), and nurses (n = 20) regarding home nursing care were studied using a descriptive survey conducted over a 6–month period in a large, voluntary, nonprofit home health agency. An original instrument, based on the American Nurses Association standards of home nursing care within a nursing process framework, was developed for this study. Factor analyses (varimax rotation) yielded four subscales: technical, professional, communication/psychosocial, and teaching aspects of nursing care. Overall, patients and physicians rated their satisfaction with nursing care identically (20% were satisfied, 71% were undecided, 9% were dissatisfied), while 70 percent of the nurses were satisfied, 20 percent were undecided, and 10 percent were dissatisfied with their nursing care. Both physicians and patients were most satisfied with professional aspects of nursing care; nurses were most satisfied with teaching aspects. Patients and physicians were most dissatisfied with teaching; nurses were equally dissatisfied with technical skills and communication/psychosocial aspects of nursing care.

摘要:
Nurse executives are charged with creating a workforce that provides quality care in a creative and cost-effective manner. Style of leadership determines how nurse executives relate to their professional nurses and ultimately how successful their health care institutions will be. The practice-based theory of nursing leadership called the Integrated Leadership Practice Model can be practiced by nurse executives to promote employee satisfaction, assure that quality care is provided based on standards and competencies, and promote an organizational culture committed to quality. This article proposes the application of integrated leadership to the practice of nurse executives and demonstrates how they can foster and support this model for nursing leadership within their nurse managers.
