摘要:
There are many functions in an integrated delivery system charged with focused, clinical excellence and process improvement based on data. Information system complexity, enhanced technology and software, and increased demand for data have created the need for one healthcare organization to look at an alternative system for data support of these key functions. This article describes a clinical data department model that has been implemented successfully in an integrated delivery system. It describes the structure and function of this department and its relationship in supporting the areas where clinical data is needed.

摘要:
End-of-life decisions affect patients and their families at critical, inevitable points in the life continuum. State-of-the-art technology and pharmaceuticals make it possible to extend life and, in many cases, prolong the process of dying. Timely and compassionately provided information empowers patients and families to make informed end-of-life decisions and avoids painful, forced decisions during such critical times. Nurses can provide information that patients and families need and want. A need exists for more comprehensive training and development to enhance nurses' involvement and effectiveness with regard to educating patients and their families about end-of-life care options.

The article reports on an exploratory descriptive study conducted to establish quality-of-care dimensions from African Americans' perspective, using a combined focus group interview and modified Delphi process. Descriptors of quality, which were identified using participants' vocabulary and thinking patterns, were used to establish quality-of-care dimensions; they were then examined in terms of an adapted quality-of-care framework. Themes and issues related to quality of care also were identified. The article analyzes the qualitative themes abstracted from that discourse and relates focus group interview guide questions to the emerging quality-of-care issues. An interpersonal processes-of-care framework is used to examine the quality-of-care themes, and the quality-of-care issues are discussed using a systems theoretical perspective.

摘要:
This exploratory study sought to test the relationship between nurses' job satisfaction and inpatient satisfaction with quality of nursing care in Taiwan. Patient and nurse data were collected from six inpatient care wards in a teaching hospital. Statistical results showed that nurses' general job satisfaction, general feelings of happiness when they are doing their jobs, and their job satisfaction are significantly correlated with at least one of the inpatient satisfaction factors studied, except for the factor of patient education. The results provide partial support for a linkage between nurses' job satisfaction and inpatient satisfaction with nursing care.

With the global emphasis on quality care based on evidence, this pilot project aimed to examine changes in nursing practice following dissemination and implementation of clinical guidelines. The project implemented a fall prevention guideline in an acute care hospital. Fall preventive nursing practice of the hospital was compared before and after implementation of the guidelines. Results about the changes in nursing practice are discussed in terms of the methods and processes of dissemination and implementation, as well as the features of the guidelines.

摘要:
Quality of nursing care can be measured by patient satisfaction within a health care institution. Patient satisfaction incorporates the needs of the patients and the goals of their healthcare provider. Nursing and patient's perception of its care should include anticipatory guidance, patient involvement and mutual agreement of what the final medical goal should be. A better understanding of maternal healthcare needs by nurses will lead to swifter maternal recovery and functioning. Communication and the development of common goals between the patient and health care provider, which includes the opportunity for pain relief, will lead to greater patient satisfaction and compliance with the health care process and its procedures.

摘要:
Caring for patients with chronic illness in an era of cost constraints and performance monitoring has led to a sharp growth in "disease management" efforts by health systems utilizing internal innovators or outside firms. This paper describes surveys and site visits of the chronic disease management activities of 72 programs nominated by experts in the field of chronic illness care as being particularly innovative and effective. The survey and analysis were guided by a Model for Effective Chronic Illness Care derived from a process of literature synthesis and expert review. The model proved to be useful in describing the characteristics consistently shared by successful programs, and the surveys indicated common barriers to further expansion of innovative pilot programs. The survey indicated that most of the nominated programs were limited in their effectiveness and reach by their reliance on traditional patient education, rather than modern self-management support, poor linkages to primary care, and reliance on referrals rather than population-based approaches.
