摘要:
The Post Anesthesia Care Unit (PACU) visitation program of Memorial Sloan-Kettering Cancer Center needed revision. The process was disorganized, visits were unescorted, and nurses and physicians were resistant. A committee was formed with the goal of developing a new unit philosophy and improving the overall process. Committee steps included reviewing the literature, educating staff about the benefits of visitation, empowering nurses to incorporate family visitation into patient care, and providing family education. As a result, staff attitudes changed, and nurses now contact the family within 90 minutes of the patient's arrival to the PACU to develop an individualized visitation plan. Morale is high, and nurses take pride in meeting the needs of patients and families.

摘要:
Cancer surgery causes stress and anxiety for families and patients. Knowing that communication between family and health care professionals is traditionally fragmented during the perioperative period, Memorial Sloan-Kettering Cancer Center developed a family-centered program to improve information exchange. This article describes the Perioperative Clinical Nurse Specialist program and the process by which ongoing communication of clinical information and psychosocial support for waiting relatives occurs. Good communication during the preoperative, intraoperative, and postoperative period is characteristic of quality care at a critical time. The subject program serves to bridge the gap in the continuum of perioperative care.

摘要:
The popularity of complementary and alternative medicine (CAM) has increased tremendously in recent years. Thus, it is imperative to distinguish between alternative therapies that can be dangerous and complementary therapies that are primarily palliative and augment conventional treatment. Memorial Sloan-Kettering Cancer Center's Integrative Medicine Service offers complementary therapies to patients in an attempt to improve quality of life and provide symptom management. In addition to clinical services, it also provides education to health care professionals and the public and performs clinical and laboratory research on complementary modalities and the antitumor properties of botanicals. If CAM is to be accepted by mainstream medicine, research must be done using standard research methodologies.

Cancer and cancer therapies cause side effects that prevent patients from consuming adequate amounts of food. As evidenced by patient meal consumption studies conducted at patients' bedside, only 39 percent of patients surveyed consumed greater than 50 percent of their main entrée. The Food and Nutrition Services Department at Memorial Sloan-Kettering Cancer Center used the organization's performance improvement process, STRAIGHT-A, to develop and execute a room service program designed to increase patient meal consumption and improve patient meal satisfaction A multidisciplinary task force developed policies and procedures and tested new menu items as part of the room service plan. After program implementation, 88 percent of patients surveyed consumed greater than 50 percent of their main entrée.

Benchmarking, while a useful way to compare outcomes among health care institutions, has been less useful for institutions dealing with specialty patient populations such as cancer, rehabilitation, or psychiatry. Because of regulatory requirements mandating the use of benchmarking for accreditation and performance improvement purposes, a group of comprehensive cancer centers developed a specialized database for benchmarking outcomes for cancer patients. This article describes the development of the database and some of the obstacles encountered by the group. It also outlines solutions to the obstacles.

Oncology patients requiring outpatient services have expectations that must be addressed to ensure continuous and improved quality of care. A convenience sample of 96 patients recruited from an oncology outpatient center completed a 26-item patient satisfaction questionnaire (SEQUS). Overall satisfaction ratings indicate that patients are satisfied with their care. Patients' perception of waiting time and lack of questioning regarding their medications by the pharmacist were identified as two areas needing improvement. Findings suggest that by identifying what is most important to patients, nurses can readily modify the care environment to enhance patient satisfaction and quality of care.

A four-pronged approach to improving the blood product delivery system resulted in a significant decrease in the number of blood products ordered STAT, increased ability of the blood bank to fulfill STAT requests in a timely manner, and a significant decrease in the number of blood products outdated and discarded.

Cancer survival has improved, and more attention is now being focused on quality of life. Quality-of-life issues include the physical, psychological, social, spiritual, and financial aspects of caring. Because patient support services can play an important role in helping patients cope with their disease, they are critical to patients with cancer and their families. This article describes the methods used to address the need for patient support resources and identifies the process for identifying and evaluating existing resources and developing additional ones. The project goal was to streamline patient care access across the oncology care continuum from prevention, diagnosis, and treatment to end of life in ambulatory, hospital, and hospice settings.

摘要:
Patients, consumer advocacy groups, and regulatory and accrediting bodies have spoken out about the importance of patient safety in the health care environment. Understandably, patient safety is a leading concern for both consumers and health care providers. These efforts have encouraged health care organizations and providers to review and revise, as necessary, routine practices that contribute to a culture of patient safety and avoid medical errors. This article outlines the process and outcome of a performance improvement initiative to develop a standard patient identification policy.
