Studies show that as many as 40 percent of all patients in general hospitals are admitted because of complications related to alcoholism. However, the literature has little in specific clinical pathways, treatment protocols, or guidelines for the interdisciplinary care of these patients in the acute care setting. Furthermore, the little information that is published shows a lack of consistency in recommended treatment regimes. This article reviews a coordinated, interdisciplinary effort in developing and implementing a clinical pathway for alcohol withdrawal.

摘要:
Clinical pathway teams are one type of interdisciplinary team formed to improve the quality of care provided to select patient populations. As they aim to improve quality, these teams look at improving the efficiency and effectiveness of the care provided, as well as overall performance. Some clinical pathways and teams are successful, while others are not. This article describes the activities of a coronary artery bypass graft (CABG) clinical pathway team, difficulties encountered by the team, and strategies developed to overcome these difficulties.

摘要:
This exploratory study was undertaken to discover the defining dimensions of nursing home care quality and to propose a conceptual model to guide nursing home quality research and the development of instruments to measure nursing home care quality. Three focus groups were conducted in three central Missouri communities. A naturalistic inductive analysis of the transcribed content was completed. Two core variables (interaction and odor) and several related concepts emerged from the data. Using the core variables, related concepts, and detailed descriptions from participants, three models of nursing home care quality emerged from the analysis: (1) a model of a nursing home with good quality care; (2) a model of a nursing home with poor quality care; and (3) a multidimensional model of nursing home care quality. The seven dimensions of the multidimensional model of nursing home care quality are: central focus, interaction, milieu, environment, individualized care, staff, and safety. To pursue quality, the many dimensions must be of primary concern to nursing homes. We are testing an instrument based on the model to observe and score the dimensions of nursing home care quality.

摘要:
Mislabeled umbilical cord blood specimens were identified as an important and difficult problem to solve. Quality improvement principles were employed after education-based interventions failed to achieve measurable improvement. A small interdisciplinary working group of key stakeholders investigated, designed, and evaluated interventions for a solution. This article describes a system-based change where substantial qualitative and quantitative improvements were measured. The success of the change is attributed to the involvement and commitment by key stakeholders and use of systems reengineering principles.

When evaluating the effectiveness of a fall prevention program, it is useful to first determine whether the program is being uniformly administered. Members of a medical-surgical unit's Quality Assurance/Product Improvement Team studied both processes as well as outcomes over a two-year implementation of a fall prevention program. They discovered that initial attempts at implementation underestimated the quantity of resources necessary to ensure full implementation of the program. This article chronicles the series of audits and program modifications that eventually brought about success to this unit.

Patient education has always been an integral part of quality patient care. Effective patient education is a necessary component of patient satisfaction and has proven instrumental in decreasing hospital days while improving clinical outcomes. Joint Commission and state public health departments regard patient education as one of many organizational performance standards. To be effective, health education must be coordinated among all disciplines involved in the continuum of care. The Patient Family Educational Advisory Committee (PFEAC) directed its efforts in identifying common patient education issues. The committee's initial work focused on areas dealing with culture, documentation and competency.
