摘要:
As the practice of geriatric rehabilitation grows, so does the importance of understanding the psychosocial theoretical constructs that frame our practice. Erik Erikson sets forth the conflict between integrity and despair as the eighth and final developmental task of the lifespan. The adaptive strength that emerges from this struggle is wisdom. Erikson's developmental approach is examined as it relates to rehabilitative interventions in the geriatric population, with particular focus on use of imagination, acknowledging the potential value of patients' dysfunctions, and attachment. Treating patients in the larger context of Erikson's psychosocial theory is explored as a means to achieve optimal physical rehabilitation outcomes.

摘要:
This article offers an approach to the assessment of frail older adults with complex needs that has been used by the University of North Carolina at Chapel Hill Program on Aging as both a care delivery and a teaching strategy. The patient is assessed simultaneously by different disciplines to develop a holistic and practical plan of care. It is termed "arena assessment," borrowing from the pediatric literature on transdisciplinary assessment of children. The article explores the benefits and limitations of this approach for patients and families, as well as for professional learners. Case examples illustrate use of the model from community-based to institutional care.

摘要:
Older adults in long-term care are at great risk of injury from falling. An interdisciplinary fall prevention team recommended four classes of interventions for 25 long-term care residents with significant fall histories. Data were collected covering 90-day, pre- and post-intervention time frames. Resident falls evidenced a 39.8% decline (p= .0001). Resident injuries declined 49% consisting of mild (p= .05) and severe injuries (p= .04). The findings of this study suggest that an interdisciplinary fall team is an effective tool for reducing resident falls and fall-related injuries in a long-term care setting.

摘要:
Despite the fact that protein energy malnutrition (PEM) occurs in as many as 60% of hospitalized older adults, it often goes undetected in many acute care settings. Older patients who have PEM are at significantly higher risk for functional status decline, complications, nursing home placement, and death. If recognized early and treated effectively, the adverse outcomes associated with malnutrition during hospitalization may be alleviated. Increased awareness among health care providers of the significant role nutritional status plays in reducing the negative outcomes associated with hospitalization in older patients is essential. This article reviews key nutritional screening and assessment findings important to consider for the older hospitalized patient.

摘要:
This study used the qualitative method of narrative analysis to explain rural African American caregivers' and stroke survivors' satisfaction with assistance received from health care providers and family members in regard to caregiving. The findings revealed three themes including professional (formal) care, familial (informal) care, and insufficient care. The study found that caregivers were dissatisfied with the assistance they received, and most recognized that they needed more help. The views of caregivers and older adults must be considered in order to develop interventions that increase their level of satisfaction with the care received from health and service providers.
