A study was performed to evaluate motility patterns and the role of intestinal sphincters in a diarrhea model. Fluid was infused into the canine duodenum to produce diarrhea. We discerned 4 patterns of action in the development of volumogenic diarrhea: 1. the anal sphincter was open and the ileocecal sphincter prevented diarrhea, 2. the ileocecal and anal sphincters acted independently both preventing diarrhea to some extent, 3. the ileocecal sphincter was open and the anal sphincter prevented diarrhea, and 4. diarrhea occurred when the ileocecal sphincter was closed because of spontaneous colonic motility. Pattern 3 occurred most frequently. Duplication of these patterns was accomplished with a physical analogue constructed from Penrose tubing and clamps to simulate the intestines and the sphincters.

Gastric secretory function was studied for 24 h in rats bearing permanent gastric fistulas and permanent intravenous cannulas. Rats were oophorectomized and treated for five months with testosterone or estrone, prior to gastric function studies. Gastric juice was collected for 24 h during the infusion of saline or histamine, and then gastric examined for the presence of peptic ulcers. All androgen-treated rats had peptic ulcers at the end of 24 h histamine infusion. Only 14% of estrone-treated rats had these lesions, under similar stimulation. All ulcers were localized in glandular mucosa. No ulcers were found in oophorectomized, untreated controls, infused with histamine. Volume, HCl and pepsin output were not affected by hormonal treatment. Androgen enhanced the susceptibüity of gastric mucosa to the peptic action of gastric juice. Absorption studies in a 13-year-old boy after subtotal resection of the small bowel revealed, besides the well-known malabsorption of carbohydrates, fat, iron and vitamins, a decreased absorptive capacity for sodium chloride. Following the addition of 256 mM of salt to the daily diet, the stool volume almost doubled, and half the amount of the administered sodium accompanied by high amounts of potassium were excreted in the stools. The loading of control children with sodium chloride resulted in osmotic diuresis, the additional salt being excreted by the kidneys without increasing the enteral water-sodium- and potassium excretion. Since high oral intake of salt may result following subtotal resection of the small intestine in ‘osmotic diarrhea’, the oral salt intake of such patients should not be kept high. In 20 patients with duodenal ulcer or gastric ulcer the secretion of pepsin following a continuous intravenous infusion of pentagastrin in a dose of 0.1 μg/kg/min and of histamine in a dose of 0.7 μg/kg/min respectively, has been investigated. In all cases stimulation resulted in a considerable and constant increase in the secretion of pepsin above the basal values in output as well as in concentration. The total output of pepsin was found to be slightly higher follosing administration of histamine, than following administration of pentagastrin, but this difference is not statistically significant. The maximal values of output and concentration are obtained slightly sooner following pentagastrin stimulation than following histamine stimulation, but no other differences have been found. In 32 patients with chronic renal disease the exocrine function of the pancreas was studied by the pancreozymin-secretin test. The patients were divided into two groups: 16 patients without renal failure and 16 patients with renal failure; of these, 7 were submitted to the chronic hemodialysis program. Normal function of the pancreas was found only in 28.1% of all patients. The dominating disorder was the decrease in the peak concentration of bicarbonate which was found in 50% of patients with renal failure and in 12.5% of patients without renal failure. A statistically significant correlation was found to be between the serum bicarbonate and both the peak concentration of bicarbonate and output of amylase in the pancreatic juice. Possible pathogenetic factors causing uremic pancreopathy were discussed. The impaired pancreatic function was suggested to have an importance in the mechanism of the origin of duodenal ulcer and duodenal bleeding in uremia.

The effect of short-term treatment of anabolic steroid on the patients with compensated liver cirrhosis was assessed by the study on the hepatic protein synthesis in vitro. The results indicated that the treatment decreased the specific activity of hepatic trichloroacetic acid soluble fraction and increased that of hepatic trichloroacetic acid ethanol soluble fraction. Clinical studies resulted in an improvement of serum protein pattern and BSP retention, but no effect on hepatic histology. The effect of anabolic steroid on chronic liver disease was analytically discussed.
