Enterokinase, trypsin, and amylase were measured in the intestinal content of patients with contaminated small-bowel syndrome. Enterokinase activity was also measured in the duodenal mucosa. Enterokinase activity decreased in the luminal fluid at the level of stasis and bacterial overgrowth and in the duodenal mucosa.

The size and distribution of intestinal lymph fat particles have been determined in rats fed 80 or 800 mg of various fats (corn oil, lard, margarine, butter, cream, milk). Morphological studies by means of the electron microscope were made either after fixation, inclusion and section, or after fixation and shadow only. VLDL (d < 0. 1 μm) were always abundant; generally, chylomicron diameters were 0.1–0.3 μm; larger ones were sometimes found chiefly after feeding corn oil or margarine. The presence, in the meal, of lipids other than triglycerides and of non-fat components might modify the structure of chylomicron interface.

In conscious cats with chronic gastric and pancreatic fistulas, the actions of metiamide, a histamine H2-receρtor antagonist, was compared with those of atropine. Metiamide, given in a dose of 12 μmol/kg.h caused strong inhibition of gastric acid and pepsin secretion and prevented formation of peptic ulcers induced by histamine and pentagastrin without affecting pancreatic response to exogenous secretin or to duodenal acidification. Atropine given in a dose of 0.12 μmol/kg.h significantly decreased gastric acid and pepsin secretion and prevented duodenal ulcers induced by pentagastrin but not by histamine. Atropine also inhibited pancreatic response to exogenous secretin and duodenal acidification. These experiments provide evidence that metiamide is a more potent inhibitor of histamine-induced gastric secretion and peptic ulcer formation than atropine and does not affect pancreatic response to secretin inhibited by atropine.

Resting gastroesophageal sphincter pressure was measured with continuously perfused catheters in control subjects and in patients with gastric or duodenal ulcer. Gastric acid secretion was also determined. No significant differences in resting sphincter pressure was noted in any group. No relation was found among basal or stimulated acid secretion rates and sphincter pressure.

After a careful clinical investigation (anamnesis, analysis of stomach secretion, X-ray tests, esophagoscopy with biopsies) of 45 patients with axial hiatus hernia and 20 healthy volunteers with no radiological signs of reflux, manometric analysis of the lower esophageal sphincter (LES) was performed. The manometric tests applied were: (1) 3-point manometry for measuring the reflexive pressure increase in the LES after abdominal compression and (2) the continuous withdrawal method before and after intravenous injection of 0.6 μg/kg pentagastrin (LES pentagastrin test). According to the results obtained by all methods applied, the patients with hiatus hernia could be divided into two groups. Group I consists of 12 patients with moderate clinical symptoms and essentially normal pressures and reactions of the LES. Group II comprises 33 patients with more severe symptomatology such as X-ray evidence of reflux (33), stenosis (3) or esophagitis of the lower esophagus (17), hyperacidity of the stomach (5), insufficient reaction of the LES to abdominal compression (29), reduced maximum pressure in the LES and an abnormally low response of the LES to intravenous pentagastrin (19). Essentially by means of the results of the LES pentagastrin test, group II could by subdivided into group IIA, with a normal, and group IIB with significantly reduced pentagastrin response. There was a good correlation between the reduced pentagastrin response and the exhibition of more severe symptoms of the other parameters tested. Thus, it could be shown that the LES-pentagastrin test could contribute to a further differentiation, which is important in respect to the therapeutic consequences and assist in a better understanding of the pathophysiological basis of the disease.
