24-hour basal gastric acid collections were obtained from 10 healthy, young (ages 22–27) male volunteers. The hourly secretory pattern, which described a circadian periodicity (low morning, high evening acid rates), was compared to mean plasma gastrin levels obtained at 4-hour intervals. No rhythm in plasma gastrin was detected over the 24-hour collection period. Hence, the circadian rhythm of basal acid secretion in man does not appear to be related to fluctuations in plasma gastrin level

The indirect fluorescence method has been used to detect and localize carcinoembryonic antigen (CEA) in specimens of colonic carcinoma, other malignant or benign tumours, and non-cancerous tissues. The antigen was found in 39 specimens of colonic or rectal adenocarcinomas with a characteristic location at the apical pole of the cancerous cells. On the other hand, CEA was not detectable in two samples of anaplastic colonic carcinomas. Furthermore, CEA was occasionally found in some other digestive or non-digestive carcinomas. It was also present in 19 benign colo-rectal polyps examined and in the majority of 85 morphologically non-malignant colonic glands obtained from specimens excised for cancerous or non-cancerous diseases.

Serum CEA was measured in 78 patients with chronic inflammatory bowel disease. One of 17 patients with ulcerative proctitis, 3 of 30 with ulcerative colitis and 4 of 31 with Crohn’s disease had elevated serum levels of CEA. All patients with increased levels had symptomatic disease. There was no evidence for the presence of carcinoma of the colon in these patients. It is unlikely that single measurements of serum CEA by presently available tests will be of value as a screening procedure for early carcinoma of thecolon in the presence of chronic inflammatory bowel diseas

The antigenic acid-stable esterase produced by the surface mucus-secreting cells of gastric mucosa has been quantified in basal and histalog-stimulated fractions of gastric juice from 23 patients and 7 controls, using the single linear immunodiffusion technique. In controls, the mean output of the esterase was 121 ± 56 mg dry weight per hour during basal secretion; during the stimulation period, the output was 571 ± 184 mg/h. The authors compared by X-ray the passage of a milk containing lactose with that of a lactose-free milk in subjects after partial gastrectomy who, after operation, developed milk intolerance. In the X-ray picture of subjects with a lower mucosal lactase level, typical differences between the two passages are found. The main difference is that the gastric stump is unable to slow down the milk supply sufficiently to the small intestine and, thus, as a result of the lower mucosal lactase level, the relative lactase insufficiency becomes manifest. During the fractionated administration of the lactose containing milk mixture the X-ray intolerance changes disappear. When the X-ray pictures after both milk mixtures do not differ, milk intolerance has obviously another cause than lactose maldigestion.

Phenobarbitone in doses causing significant increase in bile flow had no effect on plasma bilirubin levels in the rat, nor on biliary excretion of endogenous bilirubin. In patients with Gilbert’s syndrome small doses of phenobarbitone lowered bilirubin levels without any accompanying change in BSP Tm. The action of xylamide on the gastrointestinal tract, activated by electrical or chemical stimulation, was studied in vitro and in situ in the guinea pig, cat and dog. The pAx values of the drug against various agonists were very low, suggesting that the antagonism is unspecific. The differences or the interference of xylamide with cholinoceptor, adrenoceptor or ganglion blocking agents, and with substances acting on adrenergic nerves or axonal conduction were investigated. Xylamide induced a myolytic action with a direct interference with the gastrointestinal smooth muscles.
