Serum gastrin was measured by radioimmunoassay I patients after parietal cell vagotomy for active duodenal ulcer, with and without a drainage procedure, in both the basal state and following a standard protein meal. Results were compared to those of unoperated patients who had the same standard meal combined withcontinuous intragastric neutralization. In the patients with parietal cell vagotomy alone, serum gastrin rose from a basal level of 47 ± 3.4 pg/ml to a peak of 165 ± 9.1 pg/ml after the meal. In the parietal cell vagotomy with pyloroplasty group, serum gastrin rose from 42 ± 1.2 pg/ml to a peak of 173 ± 10.9 pg/ml. In the unoperated patients undergoing concomitant intragastric neutralization, serum gastrin rose from 10 ± 0.8 pg/ml to a ‘new’ basal level of 47 ± 2.5 pg/ml and then to a peak level of 166 ± 5.1 pg/ml after protein. Neither the basal levels nor those after protein were significantly different in the three groups studied. A prospective study of 9,864 male adults in Dakar, who were bled at four-monthly intervals, defined how clinical symptoms and α-Foetoprotein positivity in the serum relate in time. Nine cases of hepatoma were identified, of which seven could be examined repeatedly till death; one remained negative, three became positive the appearance of symptoms and three were positive before theappearance of clinical signs. Two different precipitin reactions involving patient’s sera as antibody source, and proteins present in rabbit anti-fetoprotein (as well as in normal rabbit serum) as antigen are described. The aetiology of these unexpected antibodies could be established in one case (PJ serum); the heteroprecipitins were due to several previous injections of purified horse Liver cirrhosis anti-tetanus toxin sharing with rabbit serum a common antigen. In the other case (CHA), possible aetiologies are envisaged. Although such reactions are rare, their possibility must always be kept in mind, and a positive control must systematically be used in Ouchterlony reactions. 1,2-Dimethylhydrazine(DMH) was administered s.c. to a group of 20 inbred BD-IX rats at the dose of 15 mg/kg body weight, weekly for 7 months. Intestinal adenocarcinoma have been found in all of the treated animals, often associated with hyperplastic or dysplastic lesions. The intestinal cancers gave metastases in 14 animals. The low incidence of extra-intestinal malignancies, the relatively short induction time, the similarity to human colorectal adenocarcinoma, make the DMH-induced intestinal cancer a highly effective experimental model.

Human gastric juice has the ability to maintain iron in solution at elevated pH. An iron-binding substance has been isolated from pooled normal gastric juice by a process involving removal of sulphated glycoproteins by complexing with cetylpyridinium chloride, precipitation with acetone, chromatography on DEAE cellulose and gel filtration on Sephadex G-200. The results of acid hydrolysis revealed that the iron-binding substance was a glycoprotein containing approximately 90% sugar residues and 10% amino acid residues. Five sugars and 14 amino acids were identified. This glycoprotein was found to be indistinguishable from soluble blood group substances.

The nature of the interaction between a gastric iron-binding glycoprotein and iron has been studied. These experiments showed that much of the iron could be separated from the glycoprotein by the physical methods of filtration and centrifugation. Magnetic susceptibility measurements were compatible with the presence of polymerized ferric hydroxide in the iron glycoprotein complex and such iron particles were subsequently visualized by electron microscopy. Spectroscopic studies failed to show any absorption peaks or shifts which could be attributed to the chemical interaction of iron and some group on the glycoprotein. Hence it was concluded that the glycoprotein exerted its influence by maintaining colloidal iron particles in suspension. Experiments in rats testing the absorption of iron from the iron glycoprotein complex showed that the iron was appreciably better absorbed from the complex than from an acid solution of ferric chloride. Furthermore the absorption of iron from the iron glycoprotein complex was of the same order as absorption from neutral solutions of iron-containing ascorbate. The possible role of this gastric glycoprotein in iron absorption is discussed.

An analysis of the microscopical appearances of the liver in 18 patients dying of fulminant hepatic failure showed distinct differences according to the likely cause and duration of survival. In particular, mitotic figures were observed only in those dying early in the course of the disease and hyperplasia of surviving cells with nodule formation only in those who lived for longer than 10 days. A significant proportion of the cells comprising the mononuclear cell infiltrate were demonstrated to be pyroninophilic. Such cells were present more frequently in those with presumed viral hepatitis than in those with toxic liver damage from paracetamol or halothane and constitute further evidence in favour of the view that immunological processes may play some part in the pathogenesis of the hepatic necrosis.
