Abstract:Tenascin is an oligomeric glycoprotein of the extracellular matrix synthesized during embryonic development. It is prominently expressed in a variety of tumors. The role of tenascin in liver tissue is, however, unknown. We used immunocytochemistry to define the localization of tenascin and compare this with the localization of non‐collagenous proteins, such as laminin and fibronectin, in normal human liver and pathological liver from patients with chronic hepatitis, liver cirrhosis and hepatocellular carcinoma. In normal liver, tenascin expression was localized along the sinusoidal and vascular wall. In fibrotic liver, tenascin was also observed in the region between the hepatic parenchyma and the fibrosing portal tracts, especially in areas of piecemeal necrosis in chronic hepatitis. Immuno‐EM study of liver tissue in chronic hepatitis strongly suggested the synthesis and secretion of tenascin by fat‐storing cells into the space of Disse. In hepatocellular carcinoma, tenascin was expressed in both the capsule and lobular septa, but not in the sinusoidal walls of the tumors. Abstract:The aim of our study was to analyze if intraperitoneally administeredE‐coliendotoxin (1–7 mgkg‐1) causes an increase in liver malondialdehyde (MDA) and conjugated dienes, 24 h postinjection. Blood glucose, white blood cell counts, liver edema and mortality were also measured. The results of our study show that there were no signs of increased MDA or diene production in the liver. To our surprise, blood leukocyte counts of the experimental groups were not statistically different from controls. In the endotoxemic animals there was a slight liver edema which, however, did not influence the measurements of the lipid peroxidation metabolites. Blood glucose levels were decreased in the endotoxemic animals. There was no mortality in our study, perhaps due to the fact that the rats were fed and not starved. We conclude that, in our study, a bolus dose of endotoxin i.p. does not cause any signs of increased liver lipid peroxidation 24 h postinjection. Abstract:To examine the significance of fibrous septa of the liver for hepatic circulatory disturbance, haemodynamic changes were investigated in rats with septal fibrosis induced with horse serum injections. The fibrotic liver showed thin fibrous bands originating in the vicinity of the peripheral branches of the hepatic vein connected with each other and partly with the portal triads, without conspicuous periportal, pericentral or perisinusoidal fibrosis. There was no evidence of hepatic cell enlargement, disarrangement of hepatic cell plates or narrowing of the sinusoids in the fibrotic livers. Portal vascular resistance, bile production and hepatic oxygen consumption, which were measured by an isolated liver perfusion method, were much the same in the normal and the fibrotic livers. Moreover, there was no significant difference inin vivoblood pressures of the portal vein, the terminal portal venule, the terminal hepatic venule and the inferior vena cava between the normal and the fibrotic rats. These data suggest that septal fibrosis in itself does not disturb hepatic circulation.

Abstract:To clarify whether immunological heterogeneity exists among liver macrophages and whether this possible heterogeneity corresponds to varying degrees of phagocytic activity, we fractionated liver macrophages according to size via centrifugal elutriation and studied their ability to ingest latex beads, to express Ia antigen and to secrete interleukin‐1 (IL‐1)in vitro.Larger liver macrophages were found to be more active in ingesting latex beads than smaller ones (p>0.01), while smaller liver macrophages were more active in Ia antigen expression and evidenced greater fluorescence at cytofluorometric examination. In addition, smaller liver macrophages elaborated more IL‐1 than the larger liver macrophages (p>0.05). Abstract:Monokines play a major role in the regulation of hepatocyte functions. To document a possiblein situproduction of these mediators under physiological conditions, expression of IL‐1β and of IL‐6 genes was analyzed byin situhybridization in four histologically normal human livers. We detected cells containing IL‐1β mRNA or IL‐6 mRNA in all cases. Cells expressing either the IL‐1β gene or the IL‐6 gene were found with equal frequency and were similarly distributed. Although present in all liver compartments, they were selectively enriched in portal areas, in which they were detected both in endothelial positions and in perivascular connective tissues. Few positive cells were observed in hepatic lobules, most of them being located in the walls of centrolobular veins, in an endothelial position. Subcapsular cells were also shown to express monokine genes. The location of positive cells and their pattern of labelling suggested that macrophages, fibroblasts and endothelial cells were the main cell populations expressing monokine genes. In contrast, Kupffer cells, biliary epithelial cells and hepatocytes did not express monokine genes. No marker of immune activation other than monokine gene expression was detected in these histologically normal livers. Abstract:Hepatocellular carcinoma has rarely been reported in Wilson's disease, particularly in women. We describe the case of a female patient who was diagnosed with Wilson's disease at the age of 39 years, after presenting with severe neurological symptoms. She had significant neurological improvement following penicillamine therapy and succumbed to hepatocellular carcinoma at the age of 72 years, following 33 years of penicillamine therapy. The patient described here was the oldest and only the third female patient with hepatocellular carcinoma complicating Wilson's disease to be reported in the literature.

Abstract:Three hours after hysterectomy the rate of urea synthesis doubles in rats. At the same time the increase in catecholamines is at a maximum, suggesting that catecholamines might be of regulatory importance. The effect of exogenous epinephrine on the rate of urea synthesis was studied in rats receiving epinephrine at rates of 2 and 20 μg/kg/h. The low dose increased plasma concentration of catecholamines two‐fold over control values (p>0.01), comparable with the increase seen after surgery, and the high dose of epinephrine increased the concentration fivefold. The high dose increased the rate of urea synthesis by 30% (p>0.05), whereas the low dose had no effect. Following a high dose of epinephrine, alanine decreased from 358 ± 29 to 254 ± 17 mmol/1 (p>0.05), indicating that the increase in urea synthesis was due to an effect on the liver rather than on extra‐hepatic tissues, in that more aminonitrogen was eliminated from plasma than released into it.
