摘要:
Identification and measurement of the outcomes of nursing care have received great attention in the past decade. With advances in information technology, large clinical data bases are being developed for use in outcomes evaluation. It is important that nurses be thoughtful in designing the data elements that will be included in these data bases. Key data elements include nursing diagnoses, interventions, and nursing sensitive outcomes. Multiple variables influence the outcome of health care. It is important in evaluation that system, patient, and provided characteristics be considered, especially when developing benchmarks for comparison of outcome achievement. Data related to the process of care are needed beyond provider encounter, and nurses must be cautious that the care they give is not credited to other providers. Longitudinal clinical data bases that contain key variables related to the delivery of nursing care can yield valuable knowledge on the best methods to measure and interpret the contribution of nursing care to the health of the nation.

摘要:
An overriding concern of many health care providers is how to maintain quality in such a turbulent, cost-driven environment. It is essential for health care providers to determine the effectiveness of their care and to institute changes in practice to affect patient outcomes in a positive fashion. Determining the appropriate outcomes to measure and formatting the report in an easy-to-read manner with meaningful information is important for presentation to a variety of audiences. We describe a report card methodology for disseminating outcome information that can easily be adapted in other settings.

Patient satisfaction has become one measure of quality of nursing care. This article reports on two preliminary studies and one major study of the Patient Satisfaction with Nursing Care Questionnaire. This questionnaire includes four general questions related to the patient's satisfaction with the overall stay in the hospital, food provided, and medical and nursing care received and 15 items that measure satisfaction with three dimensions of nursing care: caring about patients (interpersonal), technical skills, and patient education.

Client satisfaction and pregnancy outcomes of two groups of low socioeconomic women were compared. One group received prenatal care from a public health clinic, the other from a prototype tertiary multidisciplinary clinic. Sixty-two women from each clinic completed the Risser Patient Satisfaction instrument, after no less than three clinic visits. Clients from the multidisciplinary clinic were significantly more satisfied on three variables related to nursing care (technical skills, teaching, and interpersonal relationships), and one variable related to setting. Postdelivery log reviews revealed that infant birth weights, Apgar scores, and gestational ages were not significantly different between the two groups. Nurse case managed care and removal of barriers contributed to more satisfied consumers. Continuity of prenatal care was associated with healthy infants regardless of client perceptions of satisfaction.

摘要:
Managed care has ushered in a whole new way of providing patient care. This era of health care delivery is heavily focused on the attainment of defined outcomes. While many types of outcome indicators exist, the types gaining in importance are related to patient care. This article examines the concept of patient outcomes and relates them to an actual example of one condition and related medical and nursing diagnoses. The importance of measurements in the overall care delivery process and the documentation of discharge outcomes are also discussed. A methodology for outcomes measurement determination is featured.

Socioeconomic status is a powerful determinant of health care outcomes. Affordability of health care probably has more effects on the treatment patients receive than do clinical factors or practitioner expertise. This article reviews and displays the research base for aspects of socioeconomic factors and measurement issues and their impact on outcomes. A strong and consistent relationship between socioeconomic factors and outcomes challenges nurses to find strategies for working effectively with vulnerable populations. The documentation of discrepancies in outcomes provides evidence of domains in which clinicians and policy makers can have measurable impact.
