For more than 50 years, the Division of Nursing has supported innovative initiatives to improve nursing practice, increase the knowledge and skills of nursing personnel, enhance their effectiveness in the delivery of health care across health care settings, and increase the number of qualified professional nurses. Nursing centers are funded through Special Project Grants and Contracts authorized under section 820 (b) of the Public Health Service Act. The grants are targeted to establish or expand nursing centers for the purpose of demonstrating methods to improve access to primary health care in medically underserved communities. Nursing centers provide faculty, staff, and students with unique opportunities to plan, implement, and evaluate nursing services, including primary health care and health promotion services that draw on the health promotion knowledge and skills integrated throughout didactic and clinical courses. Each project demonstrates the relationship of nursing center services to Healthy People 2000 objectives. The scope of services available through nursing centers addresses diverse needs of diverse populations across the entire life span. This article describes the success of nursing centers as models of population-based, nurse practice arrangements in communities where faculty and staff provide students with opportunities to incorporate such concepts as health promotion, public health, community, and advanced practice in preparation for expanded roles in meeting unmet health care needs outside of hospitals.

Nursing centers offer an opportunity to test new models of health care delivery that emphasize health promotion for individuals, families, and whole communities. The Lundeen Community Nursing Center (CNC) model is a community partnership approach to the promotion of health. Organizational principles that guide the development of this model focus on a holistic theoretical framework; continuous community assessment; collegial interdisciplinary relationships; community-wide programming; computerized health-focused clinical documentation of client issues and provider interventions: and integration of health care professional's education, clinical and health services research, and policy activities. Data from one CNC demonstration site document the extensive emphasis on health promotion activities. A series of principles that facilitate the development of community nursing centers based on this model is presented.

摘要:
This article offers a unique perspective on health promotion and the La Salle University Neighborhood Nursing Center, located in an urban area of Philadelphia. Its authors have been and continue to be intimately involved in the nursing center movement in the greater Philadelphia area and present this model as an exemplar of primary health care and health promotion in a changing urban community. Health promotion at the individual and aggregate level is described using the specific examples of immunization, lead poisoning prevention, and cancer awareness and screening.

The use of population level data is central to developing and managing community-focused programs that promote and protect the health of the public and to evaluating the outcomes of these efforts. Community nursing centers provide a focus to examine the use of population level data in the development of programs for community health practice and research. While health-related data may be analyzed at the level of individuals, families, populations, or communities, for effective community programs the use of population level data is essential. The conceptual relationship of these different levels of data to the organizational process is explored.

Cardiovascular disease (CVD) is the leading cause of death among Americans. Although efforts have been made to educate the public on CVD risk factors, gaps may still exist between information dissemination and application in people's lives. To determine if CVD knowledge is related to CVD risk-reducing behaviors, 187 adults were surveyed using a questionnaire developed to assess CVD knowledge and health-related behaviors. No associations existed between knowledge and having cholesterol and blood pressure examinations, suggesting that, in this population, CVD knowledge alone does not translate into health-related practices aimed at reducing CVD risk.

The North Hills Health Center (NHHC) is a collaborative effort involving Abington Memorial Hospital, the North Hills Community Group, Upper Dublin Township in the suburban Philadelphia area, and funding organizations. The NHHC opened as a hospital-sponsored, community-based, nurse-managed health care center in July 1996 in response to needs voiced by the residents of North Hills. The NHHC serves this Montgomery County community of low-to-middle income African American residents. There has been a steady increase in programs, services, and patients during the first 2 years of its existence.
