Family violence occurs with alarming prevalence in a variety of forms, affecting family members of all ages. Family violence prevention requires fundamental restructuring of society to strengthen the caregiving capacity of individuals, families, and communities. This article reviews the foundation for planned change, including goals, principles, and a brief overview of specific methods of preventive intervention. Core methods include social skills education, legal sanctions, tangible family resources, social support, and positive social and physical environments.

Most tobacco use rates for adolescents do not take into account high school dropouts or students involved in alternative school settings. Project IMPACT (Innovative Methods for Peers Actively Controlling Tobacco) was a tobacco use prevention and cessation program for high-risk youth that tracked 680 continuation high school students for 18 months. Tracking and attrition is of particular concern in research on smoking and drug use in youth due to the fact that those students who drop out of the original sample commonly are at greater risk for engaging in the problem behavior targeted by the intervention. Using Post Office services, third party information including place of employment and family members, telephone operator information, and school attendance staff, the project received completed surveys from 88% (n=599) of the original cohort. The project found that 50% of the students are current smokers (at least one cigarette during the 30 days preceding the survey). Further research is needed to find effective interventions for this high-risk population.

Reported alcohol and tobacco (ie, cigarette) use of 214 Hispanic migrant adolescents enrolled in the National Migrant Education Program in San Diego County was obtained. Level of acculturation was found to be related to alcohol use, ie, acculturated females had more drinks in their lifetime, and more acculturated males and females were more likely to binge drink in the last 30 days. In contrast to previous reports with Hispanic adults, acculturation was not related to cigarette use in this population of adolescents. Focus groups were conducted with migrant adolescents and parents in an effort to assess the need for alcohol and tobacco use prevention programs, and identify methods for their implementation if needed.

Undocumented migrants, largely from Mexico and Latin America, are crossing into the United States in unprecedented numbers. This article describes a family nurse practitioner (FNP) faculty clinic in a residential facility for illegal aliens. It documents, for the first time, the physical problems detected in newly arrived illegal migrants, defines the areas most in need of enhanced health compliance (dental care, family planning, safe sexual behavior, nutrition counseling, and stress reduction techniques), and offers a model for effective, low-cost health screening. The information contributes to our general knowledge of the health-illness status of illegal entrants

The effects of prenatal exposure to alcohol and other drugs in the newborn and their impact on the child's later development is a growing concern. Health, social service, and educational professionals in Iowa were surveyed about their involvement with this population. Maternal and child health clinic directors were asked to identify the number of chemically affected children under age 5, the criteria for such a determination, availability of resources for these children and their families, and educational needs for clinic staff. Findings are discussed and suggestions are made to better meet the needs of this vulnerable population

The present study evaluated the effects of an interactive puppet program aimed at reducing the incidence of violence and drug abuse in a sample of 290 African-American youths. Ethnically representative puppets presented skits on problem situations that reflected the everyday realities of study youth. One condition viewed puppet skits and toured exhibits that reinforced the learning from skits; a second condition viewed skits only; and the third condition did not receive intervention. Youth outcomes for both intervention conditions support the feasibility of using interactive puppet performances to reduce violence and drug abuse in vulnerable youths

This article describes the use of a private contract by a public health nursing program in Northwest Alaska to supply early and periodic screening, diagnosis, and treatment (EPSDT) services to children primarily of Inupiaq Eskimo heritage. Though a creative solution to the difficult problem of providing EPSDT examinations in this remote region, contracting had its drawbacks. An argument is presented to suggest that responsibility of EPSDT assessments be given to the first-line health care providers in the region's villages, the community health aides/practitioners. Why this has not happened already and major barriers to effecting this change are discussed

This article reports an analysis of data involving the application of a social network intervention model using lay health advisors (LHAs). The study population included migrant farmworker women and infants receiving care at two health care delivery sites. Changes in knowledge, health status, and behaviors were assessed. Our findings indicate that the LHA model is a valid one for migrant farmworker families.
