SummarySexual problems are,par excellence, psychosomatic conditions and none more so than loss of libido. However, I will be dealing with the organic rather than the emotional. My second disclaimer is that I am going to consider more about loss of libido in women than in men, partly because of my background as a gynaecologist and partly because there is considerably more written about loss of libido in women. Libido is complicated and we do not understand much about its origins, even though we are aware of many of the factors which may alter it. I will address some of these factors and look at how we as clinicians should approach the problem of loss or lack of libido when it presents to us.

SummaryI'M A rather unusual hybrid, being a member of the Institute of Psychosexual Medicine and also a gynaecologist. I hope that this meeting will inspire the gynaecologists among us to undergo similar training, or at least give some thought to the psychosexual aspects of our job which get surprisingly little attention. I want to talk about some of my own thoughts and experiences and present some cases to illustrate how the emotions surrounding vaginal examination can be very important.

SummaryTridestra (Sanofi Winthrop; Orion Pharma, Finland) is a novel hormone replacement therapy designed to aid compliance. It involves the concept of adding a progestagen at the end of every 3 months of oestrogen treatment. This means that withdrawal bleeding occurs at three-monthly intervals. It has been shown in trials comparing 1 and 3-monthly cyclical hormone replacement that menopausal women preferred quarterly withdrawal bleeding to monthly bleeding (Ettingeret al., 1994; Hirvonenet al., 1995). Tridestra is for continuous use with no tablet-free days. Oestra-diol valerate, 2 mg, is taken for 70 days, followed by 20 mg medroxyprogesterone acetate in addition to the oestradiol for 14 days (days 71–84). Each cycle is completed with seven placebo tablets on days 85–91. to aid compliance. Tridestra is licensed for the treatment of climacteric symptoms associated with the menopause and the prevention of postmenopausal osteoporosis in women considered to be at risk of developing fractures.

SummaryI was pleased, on retirement from Public Health Medicine, to be offered three sessions in Lydia–the Genitourinary Medicine Clinic at St Thomas' Hospital–one male, one female, and one psychosexual, and expected to further my field of interest from the one attached to a family planning clinic I had already been doing for several years. The demand has gradually increased and all three sessions are now psychosexual ones. Most of my patients are male, so I was pleased to hear the last speaker mention the‘other half as the emphasis today is on the female partner.

SummaryDespite a growing interest in the psychoanalytical understanding of pregnancy there is as yet no literature regarding spontaneous abortion or miscarriage, although some attention has been paid to planned abortion. Analysis of women patients who have miscarried often reveals their sense of loss, prolonged grief and unresolved mourning many years after this event. [It should be noted that one first pregnancy in four ends in miscarriage.] Analysis of these women frequently reveals a longstanding depression, a loss of self-esteem and hatred of their female bodies which do not bear live children as their mothers did. Their self-representation is damaged.
