SummaryTotal glycosylated haemoglobin (Hb A1 (a + b + c)) was measured several times during the third trimester of pregnancy in 40 insulin-treated diabetics and the results compared with the patients' glucose control and fetal outcome. In spite of poor glucose control, 2 patients were found who had normal glycosylated haemoglobin levels throughout; both produced macrosomic babies who became hypoglycaemic. Conversely, 4 patients with apparently good inpatient glucose control were found to have abnormal glycosylated haemoglobin and also delivered abnormal infants. Poor fetal outcome in terms of macrosomic or hypoglycaemia and high cord C-peptide concentration reflecting pancreaticβ-cell hyperplasia was sufficiently well associated with abnormal glycosylated haemoglobin concentration to conclude that its measurement is a useful additional test for monitoring diabetic pregnancy

SummaryThe case of a pregnant woman with a large cavernous haemangioma of the vulva in whom caesarean hysterectomy was performed is presented. The advantages of conservative treatment during pregnancy and labour are emphasized. In the present case caesarean hysterectomy was indicated and supracervical rather than total hysterectomy was desirable. Postoperative treatment with anticoagulants is suggested to prevent thrombotic complications in the haemangioma

SummaryMaternal serum alpha-fetoprotein increases slowly over 20 hours after the intra-amniotic injection of prostaglandin E2for mid-trimester termination of pregnancy. When hypertonic urea is injected together with the prostaglandin, the rise in maternal alpha-fetoprotein is much greater and occurs much more rapidly. The increase in maternal alpha-fetoprotein is not accompanied by transplacental passage of fetal red cells. It is suggested that alpha-fetoprotein passes through to the maternal circulation as a direct result of severe fetal compromise leading to fetal death

SummaryLeukin, a cationic bactericidal protein of low molecular weight that is derived primarily from the polymorphonuclear leucocyte, was found to be present in amniotic fluid from 450 normal obstetric patients at teim. SummaryFetal heart rate and uterine contractions were monitored simultaneously at the time of amniocentesis in 46 patients where the pregnancy was considered to be‘at risk’. The study was conducted to assess the fetal heart rate response to the additional stress of amniocentesis.The usual fetal heart rate response to the procedure is a transient tachycardia. This is more likely to occur when contractions are present or provoked. A nonreactive trace is more common when there are no contractions. A record of uterine contractions during the procedure is therefore necessary for evaluation. Decelerations of the fetal heart rate after amniocentesis indicate a fetus in jeopardy likely to develop fetal distress, either as a result of the procedure or in labour

SummaryA relationship has been established between the occurrence of transient fetal bradycardia at the height of a uterine contraction, with a sudden dip of at least 40 beats per minute and rapid return, and cord entanglement round the baby. In 28 cases with‘cord involvement bradycardia’fetal cardiotocographs, 17 of the babies were found to have cord entanglement at birth.This type of episode of bradycardia was not consistently associated with meconium staining of amniotic fluid, the use of oxytocic drugs or low Apgar scores for the babies. Some of the patients with cord involvement bradycardia proceeded to develop‘variable decelerations’
