the purpose of this studywas to evaluate the association between signs of trauma from occlusion, severity of periodontitis and radiographic record of bone support. The maxillary first molars of 300 individuals were independently evaluated by two examiners for signs of trauma from occlusion, pattern of occlusal contacts and severity of periodontitis. Each site was also evaluated radiographically by an independent third examiner. The results indicated that: (1) teeth with either bidigital mobility, functional mobility, a widened periodontal ligament space or the presence of radiographically visible calculus had deeper probing depth, more loss of clinical attachment and less radiographic osseous support than teeth without these findings, (2) teeth with occlusal contacts in centric relation, working, nonworking or protrusive positions did not exhibit any greater severity of periodontitis than teeth without these contacts, (3) teeth with both functional mobility and a radiographically widened periodontal ligament space had deeper probing depth, more clinical attachment loss and less radiographic osseous support than teeth without these findings and (4) given equal clinical attachment levels, teeth with evidence of functional mobility and a widened periodontal ligament space had less radiographic osseous support than teeth without these findings.

hydroxylapatite (periograf®) was placed intoperiodontal defects around five teeth scheduled for extraction in two young adult females with excellent plaque control. On the facial surface of one tooth the material was placed in a supracrestal position. Twelve months later the teeth were extracted in block section and were examined microscopically. Hydroxylapatite crystals were seen in the histologic sections with evidence of new bone formation in juxtaposition. The hydroxylapatite was tolerated relatively well by the surrounding tissue. a “cap” of bone was present coronal and facial to those crystals placed in the supracrestal position. In some areas bone was seen attached to the root via a periodontal ligament coronal to the Durapatite crystals. The question of accidental implantation of the material into the adjacent bone versus the actual regeneration of a true new attachment was discussed.

the purpose of this investigationwas to assess the value of sulcular fluid flow (SFF) measurements in evaluation of the healing of deep periodontal pockets after scaling and root planing. Fourteen adult patients with advanced periodontal disease received supragingival prophylaxis and instructions in oral hygiene. Immediately after baseline examination, meticulous scaling and root planing were performed. One to five selected pockets per patient were monitored on Days 0, 30, 120 and 210. Subgingival microbial proportions, sulcular fluid flow, probing depth, Gingival Index and bleeding after probing were assessed and recorded. The subgingival microbial proportions were determined with darkfield microscopy.The periodontal conditions were markedly improved by Day 210 as depicted by the microbial proportions. The proportion of motile micro‐organisms was reduced from 51 % at baseline to 9% on Day 30. This result was sustained until Day 210. Healing was also evident from the changes in Gingival Index scores and bleeding after probing. Probing depth was reduced from a mean of 8.3 to 4.5 mm.Correlations between the individual means recorded for the clinical parameters and bacterial proportions were calculated for the pooled data of all four examinations. Sulcular fluid flow did not correlate with the clinical parameters: Gingival Index, bleeding after probing or probing depth. The latter three parameters correlated significantly with the microbial proportions and each other. When the pooled tooth surface recordings of Days 30, 120 and 210 were divided into two groups according to the proportion of motile bacteria (the group with18% of spirochetes and motile rods), a significant difference in the mean amount of sulcular fluid flow (P<0.01) and mean probing depth (P<0.001) could be demonstrated between the two groups.

the gingival basement membrane antigens, Type IV collagen, bullous pemphigoid antigen and epidermolysis bullosa acquisita antigen were studied by indirect immunofluorescence in 11 gingival specimens from patients with periodontitis and 2 normal gingival specimens. In the normal control gingival specimens, the antigens were all present and stained with a continuous linear pattern. In periodontitis, alterations occurred in the gingival basement membrane antigens in the apical portion of the pockets. These included thinning, interruptions, partial or complete absence involving one or more rete pegs and fragmentation. These alterations may result from the disease process or play a role in the pathogenesis.

if the vertical angulation of maxillarymolar roots could be reliably predicted, this might affect the choice of root(s) to retain in root resection procedures. In an attempt to develop this prediction, angulation of palatal roots was compared to palatal vault height. Human cadaveric maxillae with molars were sectioned faciopalatally through each tooth root to the palatal midline and photographed. Root angles and palatal height were measured on the photographs in relation to a reference line connecting buccal and palatal cementoenamel junctions (CEJ). Palatal vault heights were classified as low, medium, or high according to the perpendicular distance from the midpalatal suture to the reference line. Root angles were subtended by a line following the length of pulp canals and perpendicular to the CEJ reference.Analysis by Spearman's Rank Correlation Coefficient demonstrated a weak and nonsignificant relationship between palatal vault height and the angulation of the various maxillary molar roots. Thus, palatal vault height would not be a reliable predictor of root angulation if this were to be used as a basis for root retention.

a pathologically migrated maxillary centralincisor tooth with a poor periodontal prognosis was treated successfully with conventional periodontal therapy. After almost 4 years, an extensive periodontal lesion developed on the same tooth and extraction was recommended. The patient did not comply and the acute symptoms disappeared after the administration of 1 gm of Tetracycline a day for 2 weeks. Although no definitive periodontal therapy was done, complete resolution of the lesion occurred with healing of the structures of the periodontium and a dramatic reduction in mobility of the tooth. Antibacterial therapy could be an effective means of treating some periodontal lesions once a more exact and direct association is established between the various clinical forms of periodontitis and specific periodontal pathogens or groups of pathogens.

root surfaces exposed to the oralenvironment may present a variety of problems to dental patients. Mucogingival surgical procedures represent one approach to the management of these concerns. Procedures currently in use often require two or three wounds, and sometimes two surgical sessions. Recent observations suggest that an allograft, freeze‐dried dura mater, may serve well as a donor material in mucogingival surgery. This report describes the use of coronally positioned flap procedures to treat residual areas of recession 4 years after placement of freeze‐dried dura mater allografts.

cicatricial pemphigoid is a chronicsubepideimal bullous dermatosis which primarily involves the mucous membranes. It is a disease found almost exclusively among middleaged and elderly persons. This report describes a case of cicatricial pemphigoid with onset at age 5 and involving primarily the mucous membranes of the mouth and eyes. The patient cleared well on 40 mg of Prednisone and has been maintained on a regimen of 7.5 mg of Prednisone every other day.

a case of faint occurredin a patient undergoing periodontal surgery with intravenous conscious‐sedation in the horizontal position. The faint occurred 3 hours and 40 minutes into the procedure, immediately following the extraction of a maxillary molar that could not be salvaged due to excessive bone loss. Since the patient was judged to be adequately sedated by both the operator and the anesthetist, the faint could not be attributed to anxiety. Recent evidence suggests that somatosensory stimulation of the trigeminal nerve may produce a reflex parasympathetic discharge that could account for some syncopal episodes observed during dental procedures.

oral melanin pigmentationis not uncommon in some races and ethnic groups. Repigmentation of depigmented oral mucosa has been described after chemical, thermal, surgical, pharmacologic or idiopathic factors. Information on the repigmentation of oral tissues after surgical procedures is extremely limited. This communication reports on gingival repigmentation over a 7 to 8 year period following removal of gingival tissues.Pigmented keratinized gingiva was removed in two Jewish Yemanite adult males, who had moderate or heavily pigmented gingiva. After surgery, the exposed lamina propria was covered by a periodontal pack for 7 to 10 days. The tissues were then observed periodically for signs of repigmentation. Healing was uneventful and the surgically treated areas in both patients remained depigmented over the first 2 years. After 32 months, some pigmentation was found in one of the patients, and with the exception of two limited sites, the area was completely repigmented after 7 years. The surgically treated area in the second patient remained depigmented over an 8‐year follow‐up period.These observations agree with previous reports that described gingival pigmentation as spontaneous and suggest that further controlled experimental studies be undertaken to explore the biologic basis for repigmentation.
