A total of 20 test and 20 control subjects completed an 8‐week double blind clinical trial aimed at testing the therapeutic efficacy of a 7% tetrapotassium peroxydiphosphate (PDP) solution administered as a subgingival irrigant to scaled and untreated periodontal pockets. Each patient provided similar contralateral pockets with probing depths at base line of 5 mm or more (mean 6.25 mm). One pocket was selected at random for a thorough scaling, immediately following the base line examination (examination 1). The examination of each experimental site consisted of Plaque Index (P1I) and Gingival Index (GI) scores, probing depth (PD) and attachment level (AL) measurements (in mm), and a differential microscopic proportional count of certain subgingival bacterial morphotypes, namely coccoid cells, motile bacteria (not including spirochetes), spirochetes and all residual bacterial morphotypes. Test patients received coded irrigators containing the active ingredient PDP, while control patients received irrigators containing a placebo rinse similar to the test solution, but without PDP. The participants were instructed to irrigate the test sites twice a day. They were re‐examined after four weeks (examination 2) and eight weeks (examination 3) from the base line examination.The results indicated that scaling and irrigation produced a significant decrease from the initial P1I and GI scores, and the PD and AL measurements. The proportions of coccoid cells were significantly increased, while the proportions of motile bacteria and spirochetes were significantly decreased. Irrigation of unsealed pockets resulted in a modest, but significant decrease in the P1I and GI scores. No effect on PD or AL was detectable. There was no detectable difference between the therapeutic effect of irrigation with PDP vs. the placebo solution.

The relative osteogenic potentialof two different preparations of lyophilized human demineralized bone were evaluated using a heterotopic site in a rodent model. Human diaphyseal cortical bone (75‐850 μm) was demineralized according to two different protocols for decalcified bone allograft (A. H. Reddi and M. R. Urist). Equal volumes of mineralized lyophilized human bone and a proprietary hydroxylapatite served as controls. Thirty‐two Long‐Evans rats divided into four groups received subcutaneous implants of one of these four preparations. Implants were harvested at two, four, and six weeks. Histometric analysis of limited serial sections at six weeks demonstrated new bone formation by the two demineralized preparations only. The Reddi protocol produced significantly more bone formation (1.13 ± 1.21%) than the Urist preparation (0.53 ± 0.31%). Although bone induction by both the Reddi and Urist protocol was sparse within this xenogeneic system, the Reddi preparation may offer some slight advantage of greater osteogenic potential and ease of preparation. The low yields of induced bone in response to implants of human demineralized bone would limit the use of this model system in assessing osteogenic potential prior to clinical use.

An air‐powder abrasive systemwas tested among 20 volunteers, who had healthy gingiva or slight gingivitis, but no periodontal pockets. Impressions of randomly selected areas (n = 103) were taken before and after airpolishing and positive replicas were prepared for scanning electron microscopy (SEM). Gingival bleeding increased statistically significantly (p<0.001) during the treatment. Also gingival erosive changes were increased when using airpolishing technique (p<0.001). The erosive changes observed by SEM and the subjective symptoms during and after the operation positively correlated to the presence of gingival inflammation (p<0.001). It can be concluded that also the negative aspects of the airpolishing technique should be taken into consideration, especially when operating near the gingival margin.

The role of bacteriain the pathogenesis of periodontal disease is controversial. Although bacterial penetration has been demonstrated in human Periodontitis, relationships of bacteria to varying levels of periodontal health in other animals has not been well documented. In this study of beagle dog periodontium, bacteria were counted within the various tissue regions of sites which exhibited periodontal health, gingivitis, or Periodontitis.Affected sites within whole block sections from two aged beagle dogs were prepared for investigation by light (LM) and scanning electron microscopy (SEM), and bacterial counts were subjected to statistical analysis for differences.Periodontitis sites viewed with LM had more bacteria than healthy sites. Neither LM nor SEM observations yielded any other differences in bacterial counts between the levels of periodontal health. When viewed using SEM, the sulcular epithelium had more bacteria than the other tissue regions in diseased sites. Relative to the number of bacteria in plaque, however, counts from all microscopic observations were very low.These findings indicate that the presence of bacteria within the periodontium is of questionable significance in the pathogenesis of spontaneous periodontal disease in beagle dogs.

Arandomized double‐blind crossoverdesign study using 25 patients requiring bilateral gingival flap surgery was completed. The effect of rinsing postsurgically three times daily with an antimicrobial mouthrinse or physiological saline on dental plaque formation, gingival inflammation, bleeding, wound healing, and patient comfort was evaluated at 7, 14 and 28 days.The antimicrobial rinse was statistically significantly 28.9% more effective than saline at seven days for reducing plaque. It was also significantly more effective for improving wound healing at day 7 as measured by edema (p<0.04). There were no significant differences in gingival index scores or bleeding at any time period.Use of an antimicrobial mouthrinse may be an effective aid in early healing of gingival flap surgery wounds.

摘要:
Collagen membrane preparationswere manufactured with the aim of enhancing wound healing following periodontal surgery. After crosslinking by various processing methods (with ultraviolet and hexamethylenediisocyanate) and to various extents, two types of collagen (atelocollagen and tendon collagen) were implanted into a dissection site within palatal gingival tissue. The time course of healing responses was investigated histologically. Collagen implantation was found to accelerate fibrous connective tissue attachment to the root surface and inhibit apical migration of the junctional epithelium. Cross‐linked atelocollagen was superior in biocompatibility to the other collagen membranes studied.

Since igg subclasses are commonImmunoglobulins associated with the periodontium and have different biological characteristics, these subclasses were measured in gingival crevicular fluid (GCF) from periodontally active (≥2 mm clinical attachment loss within three months of sample) versus clinically similar but stable or healthy sites. A sandwich enzyme‐linked immunosorbent assay (ELISA) using monoclonal antibodies was performed to quantitate IgG subclass and albumin concentrations in serum and interproximal GCF samples from at least one each of the three disease categories from each of 20 periodontal maintenance patients. Although much variability existed among sites, mean IgG1(p<0.05) and IgG4(p<0.01) concentrations were higher in GCF from active Periodontitis areas than stable sites, even though both had similar clinical characteristics. When IgG subclass concentrations were adjusted per mg albumin, both IgG1and IgG4levels in GCF from active sites were still significantly elevated over stable areas (p<0.05). Mean adjusted concentrations in GCF were generally greater than in serum, especially for IgG4(active site GCF:serum = 24.2:1). GCF IgG4concentrations may be useful as an indicator of the immunopathological changes which occur in active Periodontitis.

Problems of patient compliance inperiodontics are evident. This study explored factors which may contribute to the degree of adherence. Using the “Health Belief Model” a questionnaire was constructed and administered to 120 patients of the Department of Periodontology, University of Frankfurt Dental School. Compliance of these patients during the hygienic phase was assessed using a bleeding index. The data set for statistical evaluation comprised 96 patients. The loss was due to missing of appointments and incomplete questionnaires. There was no significant correlation between patient compliance on the one hand and sociodemographic variables (age, sex, family status), disease parameters, and the health beliefs “susceptibility,” “barriers,” “dentist‐patient‐relationship,” and “experience with therapy” on the other hand. “Motivation,” “seriousness,” “benefits,” “experience with affected organ,” and tooth‐loss‐index were significant predictors with Spearman correlation coefficients running from 0.17 to 0.32. When the predictor variables were combined the coefficient was 0.59. This study further supports the assumption that health beliefs play a significant role in the determination of health related behavior.

摘要:
Histiocytosis x, a reticuloendothelial disorder, includes the three clinical syndromes: Letterer‐Siwe and Hand‐Schuller‐Christian diseases and eosinophilic granuloma of the bone. Twenty‐eight cases of adult onset of histiocytosis X involving the head and neck region were found in the literature during the past 20 years; 15 cases were confined to this area, out of which 13 involved jaw bones. Characteristic signs and symptoms of periodontal disease were observed in 22 out of 28 cases (79%) with the lower molar area as a predominant site.A case report of a non‐osseous chronic disseminated histiocytosis X with periodontal involvement is presented. The importance of histiocytosis X, as an entity that should be kept in mind as a differential diagnosis of a process where destructive Periodontitis is involved, is discussed.
