The purpose ofthis study was to determine if adolescents and young adults with severe periodontal disease had precipitating serum antibody reactive withActinobacillus actinomycetemcomitans, and, if so, to correlate presence or absence of the precipitin with clinical patterns of periodontal destruction. Double diffusion in gel was used to detect precipitins. Test and known positive standard sera were run in different wells against a common well containing a mixture of sonicates from two strains ofA. actinomycetemcomitans(Y4 and N27). Periodontal loss of attachment was measured with a probe on four surfaces per tooth; missing and unerupted teeth were noted with the aid of radiographs. Precipitating antibody was seen in 77% of sera from 22 subjects with localized juvenile periodontitis, 40% of 35 young subjects with more generalized severe disease, and 2% of 44 periodontally healthy individuals. About 80% of the positive reactions were with strain Y4. Within the diseased groups, there were significantly fewer involved teeth in subjects with precipitating antibody (Ab+) than among those in whose sera precipitins were not seen (Ab−). Comparisons of the distribution and severity of involved teeth between Ab+ and Ab− subjects were consistent with an hypothesis that high levels of antibody specific forA. actinomycetemcomitanshave a protective effect against periodontal destruction.

The healing responseof the periodontium was evaluated after periodontal flap and debridement procedures in patients with different levels of postsurgical plaque control. Thirty‐one sites in 19 patients were included. Measurements were performed from a fixed reference point presurgically and before reentry surgery. All reentries were performed 24 to 28 weeks after surgery. Surgery consisted of elevating an inverse bevel mucoperiosteal flap, debriding root accretions and osseous defects, penetrating into the marrow, and suturing with interrupted sutures at or near the presurgical level. All patients were recalled at least once every 4 weeks after surgery for professional maintenance. The number of postsurgical maintenance visits and plaque scores (NPI) before reentry were recorded for each surgical site. Average pocket depth at the 31 sites was 7.4 mm initially and 4.1 mm at the time of reentry. This reduction in pocket depth consisted of gingival recession, which averaged 2.0 mm, and a gain in attachment level, which averaged 1.4 mm. At no site was there a loss in attachment level. Average osseous depth of the 31 defects was 3.7 mm presurgically and 1.7 mm at reentry. In addition, there was an average crestal resorption of 0.8 mm and an average osseous fill of 1.2 mm. A significant positive correlation (P<0.001) was found between gain in attachment, osseous fill and number of postsurgical maintenance visits. A significant negative correlation was found between the amount of plaque (NPI) at the study site and both gain in soft tissue attachment and osseous fill. Multiple measurements at various points within several osseous defects revealed that osseous remodeling and fill varied significantly at different locations within the same defect.

Four tooth‐containingblocks were obtained from patients being treated for infraosseous lesions of significant depth as part of their periodontal therapy. Treatment consisted of open flap debridement and professional cleansing at least every 4 weeks. Teeth in block were removed for histologic study 4 to 6 months after surgery. Histologic evaluation of the repair process showed pocket closure by epithelial and connective tissue adhesions in the form of an elongated junctional epithelium, beneath which parallel‐oriented fibers adhered to the root for a limited distance. Apical to this adhesion, functionally inserted fibers were present. Since no cementogenesis was seen at these latter areas, it must be assumed that the inserted fibers were present before the surgery and were not significantly affected by the procedure. No significant evidence of crestal osteogenesis was noted. However, comparison of the clinical osseous profile recorded at the time of surgical debridement with the flat crest seen in two of our cases histologically, suggests that significant crestal resorption had taken place postsurgically in some of these lesions.

The ideal ofperiodontal surgery is the total regeneration of the lost periodontal complex. A promising new osseous grafting material is Dental Matrix Gelatin (DMG). DMG was prepared by a method similar to that of Conover and Urist (1979). This consisted of sequential extraction in 1:1 chloroform‐methanol, 25°C for 1 hour; 0.6 N HCl, 2°C for 24 hours with constant agitation; 2 M CaCl2, 2°C for 1 hour; 0.5 M EDTA pH 7.4, 2°C for 1 hour; washed in distilled water 1 hour. Twelve rats were anesthetized, had heads shaved, midline flaps reflected, and 2 mm holes drilled through the right and left parietal bones. This type of osseous defect normally heals only by fibrous scarring and has been used to define osteoinductive materials. The DMG was cut into pieces about 1 mm square and placed into the right side defect while the left side remained open as a control. The animals were sacrificed on a schedule of two rats every 2 weeks until the 10th week when four rats were killed. The results showed complete osseous closure of the DMG site while the control healed by fibrous scarring. DMG seems to have strong osteoinductive power, and used allogenically has great potential as a commercially viable implant material.

The roleof personal plaque control in periodontal maintenance care was studied in 78 patients who had undergone periodontal therapy and were on 3‐month recall for prophylaxis over 8 years. Variations in pocket depth and attachment levels were related to individuals with plaque scores above and below the median. The results also were analyzed by comparing the 25% of the sample having the lowest plaque scores with the 25% having the highest scores over 7 years of maintenance care. Studentsttest was used. It was found that personal oral hygiene as expressed in plaque scores was not critical for maintenance of post‐treatment pocket depth and attachment levels in patients with professional tooth cleaning every 3 months. The initial post‐treatment reductions in pocket depth and variations in attachment levels were more favorable in patients with good than with poor oral hygiene, but, these differences were not significant after 3 to 4 years of maintenance care.

The significanceof recurrent gingivitis related to maintenance of post‐treatment pocket depth and clinical attachment levels was tested in 78 patients over 7 years of maintenance care with prophylaxis every 3 months. Under the experimental conditions the severity of the recurrent gingivitis did not significantly affect the pocket depth and attachment levels.

The primary objectiveof this study was to compare the effectiveness of hand and ultrasonic instrumentation in removing subgingival plaque. Twenty‐four teeth in five patients were randomly assigned to be scaled by hand instruments, scaled by ultrasonic instruments, or uninstrumented as a control. The subgingival tooth surface was delineated by a groove, and the teeth were then extracted for periodontal reasons. The percentage of plaque area relative to the total surface area of the side of the tooth was computed. No significant difference was found between the two methods in removing subgingival plaque. Furthermore, neither method removed all of the plaque.

Electron microscopicexamination of inflamed gingival connective tissue of ligature‐induced periodontitis in the cat following a therapeutic to high regime of parentally administered soluble gold, revealed electron‐dense material characteristic of gold deposits within phagolysosomes of macrophages, and to a lesser extent, endothelial cells and fibroblasts. Since gold salts can alter some of the events that may occur in periodontal breakdown, chrysotherapy may have a beneficial effect on the pathogenesis and progression of experimental periodontitis in the ligature‐induced model of periodontal disease.

Antibodies to herpessimplex virus were found by immunofluorescence in the gingival fluid of patients suffering from recurrent oral herpetic infections. The type of antibodies was modulated by the clinical state of the patient. IgG antibodies were found before infection and secretory IgA antibodies during the convalescent stage; in the acute stage no antibodies were detectable.
