Tioconazole is a new imidazole antifungal agent with broad-spectrum activity. Its in vitro activity against common dermal pathogens is generally better than miconazole by a factor of 2–8. A total of 61 patients with proven fungal infection of the skin or with erythrasma took part in a double-blind comparison of the efficacy, toleration and safety of tioconazole base 1% w/w and econazole nitrate 1% creams applied twice daily. Some patients had more than one site of infection and some were infected with more than one fungal/bacterial species. The mean duration of treatment in the tioconazole group of patients was 40 days (range: 12–68) and in the econazole group 38 days (range: 17–101). Clinical and mycologic cure was achieved in 27 of 29 (93.1%) tioconazole-treated and 29 of 31 (93.5%) econazole-treated patients. All but 1 of the cured patients in each treatment group were seen some 6 weeks after the end of treatment; all remained clinically and mycologically cured. Both creams were found to be generally acceptable in terms of ease of application. Some staining was reported in both treatment groups. Unstimulated mononuclear cell supernatants from patients with systemic scleroderma significantly augmented the DNA and collagen biosynthesis of 3T3 cells compared to normal mononuclear cells. This increase was suppressed by either a particular glycosaminoglycan, which from previous studies, has appeared to act as one of the tissue antigens in sclerotic skin, or concanavalin A stimulation.

A total of 60 patients with proven fungal infection of the skin took part in a single-blind comparative study of the efficacy, toleration and safety of tioconazole base 1 % w/w and miconazole nitrate 2% creams applied twice daily for 14 days to patients with pityriasis versicolor and for 28 days to patients with dermatophyte and yeast infections. Overall assessment of the patients reviewed at the end of treatment showed that 23 of 30 patients (77%) treated with tioconazole and 19 of 30 patients (63%) treated with miconazole were clinically and mycologically cured. Tioconazole cured 80% (8 of 10) and miconazole cured 38% (5 of 13) of the infections with Trichophyton rubrum, which are the most frequent ones. At long-term follow-up, approximately 6 weeks later, results in both treatment groups were comparable. 20 of the 23 patients (87%) cured by tioconazole and 18 of the 19 patients (95%) cured by miconazole remained free from infection. Both creams were easy to apply and there were no reports of staining. No side effects were recorded during the study.

In 24 psoriatics of either sex aged between 22 and 84 years, functional analysis of sessile macrophages in liver and spleen revealed an accelerated metabolic elimination of Tc-99m labeled human serum albumin millimicrospheres. In psoriatics treated with aromatic retinoid, macrophage activity in liver and spleen was found to be normalized compared to the group of systemically nontreated psoriatics. 3 psoriatics with cirrhosis of the liver demonstrated an unequivocally delayed tracer accumulation in liver and splenic macrophages, the former showing decelerated phagocytosis and proteolysis of ingested albumin particles. The method used allows measuring of phagocytic and proteolytic properties of tissue-fixed macrophages in psoriatics as well as assessing the therapeutic effects of systemically administered drugs on macrophage functions.

In 32 studies involving 1,304 patients tioconazole 1% dermal cream has been shown to be effective and safe in the treatment of a wide variety of superficial fungal infections of the skin and erythrasma. Tioconazole cream is more effective than miconazole nitrate 2% cream in the treatment of pityriasis versicolor and in infections with Trichophyton rubrum and Trichophyton mentagrophytes which cause 70% of dermatophyte infections in man. Data from comparisons with econazole and clotrimazole are too few to allow conclusions to be drawn on relative efficacy. All the creams were easy to apply and there were no serious adverse reactions, local or systemic.

A clinical picture of familial continual peeling of the skin is described and discussed together with light and ultramicroscopic manifestations in one of the probands. The pattern of splitting distinguishes this condition from keratolysis exfoliativa. The ultramicroscopy exhibited mainly predominance of low-density keratocytes in the corneal layer, abnormalities in distribution of lamellar granules and disrupted coating of the cells in the intercellular spaces of the corneal layer.

Two brothers with typical, symmetrical lesions of elastosis perforans serpiginosa are presented. Familial occurrence has been reported only once. In neither family was the disease associated with heritable disorders of the connective tissue. The pedigree analysis suggests an autosomal-recessive pattern of inheritance.

A case of tuberculosis involving facial skin, nasal cavity, throat and eyes was found in a young Korean girl. We report the case because of its unusual clinical picture and to recall the morbidity of cutaneous tuberculosis in the present dermatological field.
