Basement membranes are composed of dissimilar protein subunits. The procollagen-like subunit is associated with noncollagenous matrix glycoproteins. The proportion of the latter components varies among basement membranes. The various subunits interact via hydrogen bonds, disulfide bonds and aldehyde-derived cross-links. The extensive degree of cross-linking renders basement membranes highly insoluble. A procollagen-like molecule, extracted from calf anterior lens capsule, exhibits on electron microscopy a filamentous component with a globular portion attached at one end. Treatment of basement membranes with pepsin at low temperature digests the noncollagenous glycoprotein components and allows the collagenous component to come into solution. Purification of the pepsin-solubilized collagen from basement membranes reveals a molecule composed of three identical α-chains. Other unique features include 40–50 residues of hydroxylysine, 128–140 residues of 4-hydroxyproline, 12–15 residues of 3-hydroxyproline, 29 residues of arginine, 35 residues alanine, 2–4 residues of half-cystine, 38 residues of glucosyl-galactosyl-hydroxylysine, 3 residues of mannose, 2 residues of glucosamine, and 0.3 residues of fucose. Immunochemical studies indicate the presence of three distinct antigenic components and support the evidence that one is collagenous and the other two are noncollagenous glycoproteins. The recent literature relating to pathogenesis, clinical features and management of psoriasis has been reviewed. The features of the accelerated epidermopoiesis are described and its relationship to a possible primary fault in keratinisation or dermal influences discussed. The present position on the genetics and epidemiology of psoriasis is summarised. Recent observations on the relationship of histocompatibility system antigens to ordinary and pustular psoriasis could lead to important advances. Clinical discussion is confined to psoriasis in childhood, the hair, nails and mucosal psoriasis; pustular psoriasis and psoriasis arthropathica.

Two cases of insufficiently treated early syphilis with solitary papular lesions on the penis are described. There seems to be a close resemblance between these cases and descriptions from the Salvarsan era concerning the occurrence of solitary papules located on or near the place of the healed primary syphilitic lesion, after insufficient treatment with Salvarsan. Problems in diagnosing these cases are discussed.

Syringoma occurring in two siblings in a South Indian family is reported. A familial occurrence of such an uncommon tumour is of considerable interest and might indicate a genetic influence in its causation.

The results of screening for 6-phosphogluconate dehydrogenase abnormalities in the blood of dermatological patients are reported. No differences were found between controls, psoriatics, psoriatics receiving methotrexate and a group of mixed patients. The results are discussed in relation to the effects of methotrexate on glucose-6-phosphate dehydrogenase.

High oral doses of vitamin A given to Irish rats bearing squamous carcinomas failed to produce detectable light level histologic change in the tumors. The cell line was grown in tissue culture with and without vitamin A; electron-microscopic examination failed to reveal significant differences between experimental and control cultures.

Nine patients with mycosis fungoides in different stages were treated with Bleomycin. Much better results were obtained with this new drug in the six patients with the infiltrative or beginning tumour stage than in the patients in the advanced tumour stage. Complete remission was not seen. In one case the results were objectivized by DNA cytophotometry. The role of Bleomycin in the treatment of mycosis fungoides is discussed. It is concluded that Bleomycin is not the medicament of choice for the treatment of mycosis fungoides.
