A patient presented with an infiltrate resembling scleromyxoedema Arndt-Gottron which proved to be metastatic signet-ring cell carcinoma of the stomach. Later, round infiltrates appeared on the trunk. The primary lesion was excised 9 years previously and the cutaneous involvement was the sole manifestation of the process for more than a year. Most skin metastases of gastric carcinoma fall into three groups: generalized, by hematogenic dissemination; periumbilical; and those spreading by lymphatics to neck, head and upper thorax. The morphology may in some cases give a clue as to the histological composition of the infiltrate. In our case the scleromyxoedema-like morphology was due to production of mucus by the carcinoma.

Fatal agranulocytosis developed during treatment of a patient with dermatitis herpetiformis. Although he received other medications, dapsone was considered the probable cause of the agranulocytosis. Recent reports have documented the occurrence of dapsone-induced agranulocytosis during malaria prophylaxis. The present case emphasizes the possibility of this serious and sometimes fatal complication arising during sulfone therapy of dermatologicdisorders.

Five diabetic patients developed photodermatitis following the ingestion of 250–500 mg chlorpropamide per day, from 3 weeks to 4 years after the onset of therapy. Four patients developed the rash in August and September and one in December. Four patients were treated successfully by withdrawing chlorpropamide, shielding from sunlight and administering local symptomatic therapy, while the fifth patient had to be given corticosteroids. In four patients, the skin ultimately cleared, but one patient developed a residual leukomelanoderma. Two metabolites of 14C-adenine, found by incubation with separated epidermis of hairless mice, show the same behaviour in column or paper chromatography, as those produced by the action of commercial xanthine oxidase. Therefore, also because of the characteristic ultraviolet spectrum of one of the metabolites, they can account for direct products of adenine oxidation: 8-hydroxyadenine and 2,8-dihydroxyadenine. Two cases of hypertrichosis lanuginosa in female patients with carcinoma of the colon and the rectum, respectively, are described. In 6 of the total of 9 cases reported, this syndrome occurred in association with internal malignancy.
