This paper reviews recent clinical and laboratory investigations on herpes simplex viruses (HSV). New findings add to our knowledge about factors influencing HSV infection such as site of infection, virus type and they also point to the importance of the immune system for the outcome of infection. It appears that circumstances at the time of primary infection can modify the type of immune response generated and affect the likelihood of later recrudescence. One of these factors is UV light which seems to act via a photoisomer [cis(Z-)urocanic acid] on the immune system.

The fixation of pemphigus antibodies was revealed by indirect immunofluorescence in shedding grass snake epidermis. The antibodies of patients with pemphigus vulgaris (PV) were specifically binding to the antigenic substance which appears in the snake epidermis at the initial stage of integument change. At the peak of shedding, the fixation of PV antibodies was observed only in the upper layers of the ‘old’ epidermis although the intensity of fluorescent staining was considerably reduced. At the same stage of shedding, there was marked fixation of antibodies of patients suffering from pemphigus foliaceus (PF).The specific binding of PF antibodies was noted predominantly on the cell membranes of the ‘old’ keratinocytes, localized in the upper layers of the epidermis. In contrast to bullous pemphigoid antibodies, PV and PF antibodies failed to fix in the skin of the snake prior to or after physiologic shedding. In 47 healthy volunteers betamethasone-17-valerate (0.1% solution and cream) was topically applied twice daily on the scalp skin. After 7 and 14 days treatment, anagen hairs were plucked in each volunteer from both treated and untreated contralateral scalp areas. Cell cycle analysis of plucked anagen hairs was performed by means of DNA flow cytometry. Already after topical betamethasone-17-valerate application for 7 days, cell kinetic changes were found showing a slightly significant decrease of S and G2 + M cell percentages and a significant increase of G0/1 cell percentages. The present study demonstrates the possibilities of DNA flow cytometry to study the pharmacological effects on cell kinetics of plucked human anagen hairs.

Serum samples from 401 subjects were screened for the presence of anticentromere antibody using HEp-2 cells as the substrate for an indirect immunofluorescence method. Anticentromere antibody was found in 16 cases; 8 out of 62 patients with systemic sclerosis, 3 out of 7 patients with primary Raynaud’s disease, 2 out of 41 patients with systemic lupus erythematosus, 1 out of 54 asymptomatic relatives of systemic sclerosis and 2 out of 50 patients with miscellaneous diseases. In systemic sclerosis, the patients with anticentromere antibody were limited in CREST (calcinosis, Raynaud’s phenomenon, esophageal involvement, sclerodactylia and telangiectasia) variant. Various chemoattractants have been implicated in the aetiology of the polymorphonuclear leucocyte (PMN) migration into the epidermis seen in early lesions of psoriasis. Using an underagarose technique, the in vitro chemotactic responses of PMN to the arachidonic acid lipoxygenase product leukotriene B4 (LTB4) were assayed in five groups of subjects: normal healthy volunteers (n = 12), untreated psoriatics (n = 11) and psoriatics treated with topical tar (n = 12), PUVA (n = 11) and UVB phototherapy (n = 10). No significant difference was observed between the responses of control subjects and of untreated psoriatics, nor between the untreated psoriatic group and the PUVA- and UVB-treated groups, respectively. However, comparison of the tar-treated and untreated groups revealed a significantly increased chemotactic response to LTB4 in the tar-treated group (p < 0.01).

The Muir-Torre syndrome of sebaceous neoplasms of the skin, with or without keratoacanthomas, and multiple low-grade visceral malignancies with prolonged survival is a rare disorder. Colonic polyps are frequently present, and the syndrome appears to be familial. We report 2 unrelated patients with the Muir-Torre syndrome. Each case exhibited sebaceous adenomas. Gastrointestinal findings included colonic adenocarcinomas and a tubulovillous adenoma. Although an unusual disease, the Muir-Torre syndrome requires recognition because these patients are at risk for multiple primary malignancies and may have family members also at risk.

In the present study we intended to obtain information on the evolution of near-peak blood levels during long-term treatment. 56 patients with an indication for systemic treatment of nail or skin mycoses obtained a daily dose of 200 mg ketoconazole. They were periodically checked for blood level of ketoconazole, clinical and mycological status and enzyme values. The average blood level was 2.8 ± 1.3 μg/ml plasma. The blood level was not influenced by the length of the treatment. A correlation between blood level and weight or sex was not observed, whereas a significant negative correlation occurred between blood level and age (age group 15–30 years: 3.9 ± 1.3 μg/ml; 46–60 years: 2.4 ± 0.

A possible influence of the aromatic retinoid acitretin on the hypothrombinemic action of phenprocoumon was investigated in 10 healthy male volunteers, 22–33 years of age. In a run-in period of 17 days the prothrombin complex activity (Quick) of the subjects was adjusted by individual doses of phenprocoumon (range 1.5–3 mg/day) to a therapeutic level of around 20% (INR = 3.6). Afterwards the dosage regimens were kept unchanged until day 40 when 60 mg oral vitamin K1 were administered. From day 18 to day 28 once daily doses of 50 mg acitretin were applied concurrently with phenprocoumon.The mean (± SD) prothrombin complex activity increased slightly and steadily from 19 ± 1% on day 10 over 22 ± 1% on day 18 to 24 ± 4% on day 28 and reached 27 ± 4% on day 38 (the corresponding INRs were 3.30, 2.91, 2.71 and 2.46 respectively).There were, however, no relevant changes of the Quick values in any subject which could be attributed to acitretin.
