Summary:The association between some of the major risk factors in coronary heart disease and abnormal serum lipoprotein patterns was explored in 85 patients who had suffered a myocardial infarction. It was found that hyperlipoproteinaemia was associated significantly with age less than 50 and with a positive family history of premature coronary heart disease. There was no significant association in this group of patients between hyperlipoproteinaemia and sex, obesity, smoking, diabetes mellitus, level of activity or hypertension.

Summary:Three patients who developed advanced or complete atrioventricular (AV) block as a complication of acute inferior myocardial infarction are presented. Block occurred when the sinus rate exceeded 110 to 130 per minute and 1:1 AV conduction was established below this rate. There was no associated ventricular bradycardia and thus no indication for temporary transvenous pacing. Complicating ventricular arrhythmias occurred and their management in the presence of impaired AV conduction is discussed; one such arrhythmia developed late in in the patient's hospital course, after he had left the coronary care unit. Late arrhythmias now, account for approximately one third of the deaths from acute myocardial infarction in this hospital and the provision of a subacute monitoring area following intensive coronary care is discussed.

Summary:A prospective ten‐year follow‐up study at the Cardio‐Vascular Clinic, Sydney Hospital, of patients under the age of 60 years with severe hypertension, diastolic blood pressure of 110 mm Hg, is reported. The ten year mortality rate was 51%. The adverse prognostic significance of the male sex, high diastolic blood pressure, advanced retinopathy, impaired renal function, gross cardiac enlargement, marked left ventricular hypertrophy and the presence of cardiac and cerebral complications was confirmed. Age of patient, duration of hypertension, aetiological diagnosis, positive family history and the presence of non‐specific symptoms such as head‐ache were of little prognostic significance.The five‐year mortality pattern in two consecutive groups of 100 patients first seen in 1955 and 1960 was compared. There has been improvement in the overall prognosis of hypertensive patients during the past decade. There appears to have been a reduction in the prevalence of accelerated hypertension as well as prolonged survival of patients with the acute form of the disease. Summary:Very high serum muramidase concentrations in the absence of renal impairment can serve as a marker of a number of haematological neoplastic disorders, and several cases are described where the presence of these disorders was indicated by this means before the diagnosis could be established on morphological grounds. The suitability of the muramidase assay for this purpose was supported by observations that muramidase concentrations in excess of approximately five times the mean normal value were correlated closely with either chronic granulocytic, acute monocytic or myelomonocytic leukaemia, myelofibrosis or polycythaemia vera. These neoplastic disorders appeared to be effectively distinguished by this criterion from other haematological and inflammatory disorders including reactive monocytic and granulocytic states which produced lesser elevation of the muramidase concentration.On the basis of the present experience, it is suggested that estimation of the serum muramidase concentration is particularly useful in the investigation of suspected leukaemia in refractory sideroblastic anaemia, persistent monocytosis, and pancytopenic states with cellular bone marrow where the morphological picture is difficult to interpret.

Summary:In a single blind clinical trial, a significant preference emerged for mebeverine against the placebo tablet, when patients compared the efficacy of the two agents in relieving the symptoms of the irritable colon syndrome. This result confirms a previous study and indicates that mebeverine deserves a place in the management of the irritable colon syndrome in those patients who require drug therapy.

Summary:The basal and maximal gastric acid output were determined before and during maintenance therapy with long‐acting parapenzolate bromide (Relanol) in 15 Chinese male subjects suffering from peptic ulceration. Summary:A 44‐year‐old male alcoholic patient developed severe intravascular haemolysis in association with alcoholic liver disease and hyperlipidaemia.In‐vitroincubation experiments provided evidence for a circulating factor in plasma which rendered his own and normal, compatible erythrocytes abnormally susceptible to mechanical damage. Summary:Measurement of regional hepatic blood flow using radioactive xenon by Gelin's method was studied in 20 controls and 21 patients with primary carcinoma of the liver, eight of whom had associated cirrhosis. The mean value of blood flow for controls was 28.77±9.50 ml/min/100 g, for carcinomatous nodules 12.21±5.83, for normal areas in the liver with primary carcinoma 37.88±12.88 and for cirrhotic areas in the liver with primary carcinoma 21.50±10.16. The blood flow of carcinomatous tissue is significantly less than of normal parenchyma and this can be explained by the histopathological and angiographic findings. Summary:An experimental model, originally designed to study the effects of controlled interruption of the enterohepatic circulation (EHC), has been extensively modified and adapted to study the applied physiology of an “intact” EHC of bile in Rhesus monkeys.Basically, the model consists of a surgically exteriorised extrahepatic biliary circuit in which an electronic stream splitter is interposed. This diverts every twentieth drop of bile which provides a representative sample for analysis while returning the remaining 95% of bile to the upper intestine.During these studies, the monkeys were comfortably restrained in specially designed chairs whose construction is described in detail. Animal maintenance and the design of the electronic stream splitter are also described.Analysis of the glycine: taurine and the dihydroxy: trihydroxy bile salt ratios both in gallbladder bile and in bile obtained with an “intact” EHC (by using the stream splitter), showed that the Rhesus monkey secretes a bile similar in composition to that of man.The bile salt: cholesterol: phospholipid ratios in monkey gallbladder bile (81:6:13) were also similar to those found in human bile, indicating that this animal is suitable for studies of cholesterol solubility in bile – a factor of major importance in gallstone pathogenesis.The model has been used to measure bile volume, bile salt secretion, pool size and the frequency of circulation of the bile salt pool.
