Summary:A prospective investigation of the development of hepatitis and of Australia (Au) antigenaemia was conducted in 30 unselected patients subjected to open‐heart surgery. Eleven (37%) developed post‐operative hepatitis nine of whom were icteric. Six of the eleven had Au antigenaemia.Of the 30 patients five were found to have Au antigenaemia prior to operation and this is considered to be probably the consequence of cardiac catheterisation which, as far as we are aware, has not been previously reported. This renders suspect the efficacy of the method of sterilisation in common practice. Subsequent to operation, three of the five developed hepatitis but during operation all three had been given one unit of Au antigen‐positive blood. In the remaining two, Au antigen was not detected in the post‐operative period, conceivably the result of the dilution effect of multiple transfusions. Of four other patients given Au antigen‐positive blood one developed hepatitis but three others developed antibody to Au antigen. This latter finding is considered to be an anamnestic response and is further evidence of the high frequency of infection by the Au antigen‐associated virus in the community. Summary:Combined endoscopic examination was performed in 206 patients presenting with upper gastrointestinal haemorrhage. This represents 92% of all patients admitted with this emergency to one hospital over a 12 month period.Endoscopy achieved a diagnosis of the cause of bleeding in 92% of patients. The causes were: Pyloroduodenal ulcer 32%, haemorrhagic gastritis and gastric erosions 23%, gastric ulcer 20%, oesophageal varices 6%, Mallory Weiss lesion 5%, haemorrhagic duodenitis 5%, gastric carcinoma 4%, miscellaneous causes 5%. Combined lesions were found in 12% of patients.Follow up assessment revealed endoscopy to be an accurate investigation.The mortality for the series was 4.8%. Half the deaths resulted from bleeding oesophageal varices with advanced liver disease.It is suggested that because of its accuracy and high diagnostic yield, early endoscopy may replace contrast radiography as the first investigation in upper gastrointestinal haemorrhage.

Summary:A study was made of the lungs of 13 patients who died of a respiratory illness commencing as clinical influenza during the severe epidemic of Hong Kong influenza in New South Wales in 1970.In nine patients A2/Hong Kong/68 virus was isolated from the lungs after death and in these, histological examination showed changes typical of influenza viral pneumonia. These changes appeared capable of reversal had the patients survived. In four further patients, whose clinical illness was generally of longer duration before death, there were similar histological changes in the lungs, together with more chronic changes that appeared unlikely to be completely reversible. Although virus was not cultured from their lungs and the only direct evidence for influenzal cause was a high titre of serum antibodies found in one patient during life, it was thought that viral infection was responsible for the chronic pulmonary damage in these patients.In view of these findings it is suggested that some patients who survived similar prolonged clinical illness in this or other epidemics may well have sustained irreversible damage to areas of lung. Influenzal pneumonia may therefore occasionally cause permanent pulmonary scarring and even perhaps contribute to the cause of “scar cancer

Summary:The Weed system of problem oriented medical records has been successfully introduced into a general medical unit without the need for computers or specialised stationery. The change requires an increased time commitment by resident and senior staff and the development of a training programme in case record structuring for new staff. The system is acceptable to resident staff who recognise an improvement in their own training and in patient care. A weekly clinical audit has been an integral part of the system. A quality control procedure has been developed and applied to the management of myocardial infarction. This has involved identification of problem areas and formulation of a unit policy. It is felt that this procedure has resulted in an improvement in the quality of total patient care.

Summary:A systematic survey of 88 male patients in a hypertension clinic revealed that 40 had decreased sexual potency which they spontaneously attributed to their drug therapy. No single drug was involved but the incidence was higher in more severe hypertensives on larger doses of all drugs.

Summary:The advantages of continuous EEG monitoring, during carotid endarterectomy are discussed. Observations of the types of EEG change seen when the common carotid artery is clamped are described, with the use of the EEG to indicate the adequacy of collateral circulation or the necessity for inserting a temporary shunt in the internal carotid artery.

Summary:Clinical and electrocardiographic examinations were performed on 29 individuals with X‐linked (Duchenne) pseudohypertrophic muscular dystrophy, three girls and one boy with “Duchennetype” muscular dystrophy and 19 female relatives of the X‐linked Duchenne patients. These female relatives were divided into seven definite, two probable and 10 possible heterozygous carriers of the dystrophic gene. Myocardial dystrophic involvement was manifested by the presence of significant abnormalities in 69% of the electrocardiograms in the Duchenne dystrophic group of patients. There was, however, no correlation between the electrocardiographic appearances and the patient's degree of activity, age or severity of the disease process or the presence of spinal deformities. An abnormal electrocardiogram in this group did not appear to have any prognostic significance.The electrocardiograms in the four children with “Duchenne‐type” muscular dystrophy were normal as were also all but one in the female carrier group, the only significant abnormality being minor non‐specific S‐T and T wave changes in the lateral precordial leads of one recording.
