Endovaginal sonography has become the preferred method of imaging the female pelvis. It has provided improved imaging of the ovaries and fallopian tubes and allowed more accurate diagnosis of both benign and malignant processes in the adnexa. This is a review of endovaginal adnexal imaging with emphasis on how endovaginal sonography improves the specificity in the diagnosis of specific masses of the adnexal structures.

High-resolution ultrasound of the thyroid has optimal ability to image thyroid detail and is replacing scintigraphy as the initial method of choice for evaluating the thyroid. The thyroid ultrasound is now used not only for volume determination and detection of thyroid nodules but also for characterization of nodules as benign, malignant, or dubious, being able to predict cancer with 75% accuracy when using at least three ultrasound signs of malignancy, with a true negative of 97.8%. Another major indication is for monitoring the therapeutic response of thyroid nodules when TSH suppression is induced. Ultrasound provides needle guidance for biopsies of suspicious nodules and post-surgical follow-up of patients who have previously undergone surgery for thyroid cancer.

Normal development and imaging characteristics of the placenta and membranes are discussed. Commonly observed placenta abnormalities are reviewed including placenta previa, abruption, placenta accreta, macroscopic lesions, primary neoplasms, and developmental variations, i.e. succenturiate lobes/ placenta membranacea. The umbilical cord and membranes are discussed in the second portion of this article with emphasis on two-vessel cords, umbilical cord masses, and amniotic sheets. Some of the newer sonographic techniques (translabial and transvaginal scanning) have led to improved diagnostic accuracy in several placental, uterine, and fetal conditions, and those approaches are discussed in this article, primarily with respect to placenta previa. Color and duplex Doppler sonography can often be used to increase the sonologist's confidence in a diagnosis and narrow the list of possibilities. Additionally. Doppler sonography may be helpful to improve sonographic sensitivity in the difficult diagnoses of placenta accreta or vasa previa.
