SummeryUltrasound remains important in evaluating the gallbladder and biliary tree because of its relative safety, ease of use, availability, and accuracy. Diagnostic considerations associated with gallbladder and biliary sonography are usually straightforward. Pitfalls that can confuse the sonographic interpretation can be divided into three broad groups: technical considerations, anatomic and physiologic variants, and diagnostic mimics. This analysis can be applied to gallbladder ultrasound and sonography of the intra- and extrahepatic biliary tree. It is necessary to appreciate the potential problems, errors, and differential diagnoses to minimize false positive and negative interpretations. Once identified, the pitfalls of gallbladder and biliary sonography usually do not detract significantly from the value of the procedure.

SummeryTwo-dimensional viewing of three-dimensional anatomy, using conventional ultrasound, limits clinicians' ability to quantify and visualize a number of diseases. It is one reason for the reported variability in ultrasound diagnosis. Two-dimensional ultrasound has several limitations. The 2-D ultrasound image represents a thin plane at an arbitrary angle in the body; it is difficult to localize the image plane, and reproduce it at a later time.SummeryOver the past two decades, investigators have developed 3-D ultrasound imaging techniques. The authors describe various approaches, including techniques for acquiring the necessary information to produce and view a 3-D image via computer. The various developments are discussed in regard to their applications and current limitations. Examples are given from developments of 3-D B-mode, color Doppler, and power Doppler systems for imaging various organs.

SummeryRecurrent pyogenic cholangitis is a progressive disease of uncertain etiology. Diagnosis of the disease is made in patients with chronic intermittent attacks of biliary sepsis. Imaging reveals biliary strictures and intrahepatic stones. Complications include liver abscess and cholangiocarcinoma. There is usually need for surgical treatment and repeated biliary intervention.

摘要:
SummeryDuplex color Doppler ultrasound evaluation of the extracranial carotid arteries has emerged as a cost-effective, reliable, non-invasive way to evaluate atheroselerotie disease and its complications. The accuracy of duplex color Doppler evaluation in the identification of hemodynamically significant disease can be greater than 90%. In daily practice, this accuracy can only be achieved if a variety of interpretive and technical pitfalls are overcome. The material presented in this manuscript is heavily illustrated with actual case examples obtained from retrospective correlation of the results of Duplex/color Doppler sonography and angiography. The cases provide a comprehensive visual tutorial of interpretative strategies and pitfalls.
