SUMMARYAn operation is described for the cure of severe strictures which extend posteriorly to the region of the sphincter. The operation is in two stages. Except in complicated cases, diversion of the urine is unnecessary. The length and original situation of the urethra are maintained. No tissue is excised. A suitable calibre of the reconstructed urethra is arranged and the risk of a subsequent stricture reduced to a minimum. The blood supply of all the tissues concerned is interfered with as little as possible. The complications which may occur are described, and their prevention and correction discussed.

SUMMARYThe results of four years' experience with the Bengt Johanson type of operation for repair of urethral stricture in males have been reviewed, and the relative merits of modifications favoured by various surgeons compared.This study (involving fifty‐one cases, forty‐four of which were assessable), suggests that on the whole a strict adherence to the details of the technique described by Johanson is the best insurance of consistently favourable results in a variety of hands. Morbidity and complications increased with deviation from this technique. One exception to this rule was discussed and its success thought to be due to adherence to a fundamental principle of plastic surgery– a minimum of suture‐line tension, with avoidance of ischaemia.The claims made for his operation by Johanson are substantiated, namely:–1That it can be applied to any urethral stricture or strictures.2That the principle of the buried skin strip (forming a tube by epithelialisation) is a satisfactory means of reconstructing a strictured urethra.3That post‐operative dilatations are unnecessary.4That the patient is cured of his stricture–“immediately” in a satisfactory percentage of cases, and “eventually

SUMMARYA functional partial obstruction of the ureter can be produced experimentally by clamping the ureter for six hours with Dieffenbach forceps. During the period of clamping there is severe local damage to the ureter, and this gives rise to a permanent disturbance of the neuromuscular mechanism at the site. The obstruction is incomplete, and can be compensated to allow a normal flow of urine down the ureter.If the contralateral kidney is present, no hydronephrosis develops in the kidney whose ureter is obstructed. When, however, the contralateral kidney is removed and the whole burden of excretion is thrown on the kidney with the partially obstructed ureter, hydronephrosis develops, though not so rapidly as after permanent ligation of the ureter.
