Implanted port maintenance procedures have traditionally been developed by the manufacturing companies and later adopted without question by nurses who use the ports. The skin preparation procedure published by the manufacturer of Port a- Cath™ (Pharmacia Nu Tech Laboratories, Piscataway, NJ) includes the use of sterile gloves, although many nurses in clinical settings have questioned the need for gloves. The removal of Betadine®ivith alcohol is also advocated even though the literature does not support its removal. Because no studies have been published comparing a sterile-gloved procedure with a clean, non gloved procedure for accessing ports, 40 venous ports were randomly assigned to either a manufacturerdeveloped gloved procedure or a nongloved procedure developed by a clinic niwsing staff. The ports were used in both in-patient and out-patient settings for chemotherapy and fluid administration as well as blood drawing. During the 1-year study, no febrile episodes nor septicemia occurred in either group. Three individuals had skin irritation in the gloved procedure group that was attributed to the irritation subsequent to removing Betadine with alcohol. The nongloved technique was also more time efficient and cost effective; the no-glove supply cost was $8.50 compared with the company's port access kit cost of $17.50. Replication of this study is needed as is nurse participation in product development and evaluation.
