Proper I.V. flow control requires that I.V. nurses perform what at first appears to be a paradoxical role. They must constantly incorporate new technology to the effective maintenance of I.V. flow control, while at the same time, utilize skills that have become basic to craftsmen for centuries. This presentation discusses the dynamics of interpersonal and artistic skills commonly associated with the craftsman as they relate to the technological advances brought about in I.V. flow control by physics and other sciences. The I.V. nurse is called upon daily to incorporate aspects of the old craftsman's skills with the never-ending breakthrough in science as they relate to I.V. care. Staying current with the new advances at the expense of the interpersonal and artistic skills of the craftsman is not enough. The I.V. nurse must do both.

摘要:
Total parenteral nutrition (TPN) has advanced nutritional care of hospitalized patients since it was first introduced by Dudrick in 1968. TPN consists of hyperosmolar solutions delivered through a catheter into the superior vena cava for extended periods. Patients on TPN are at risk for catheter complications of three types: 1) complications of insertion, 2) thrombosis of the great veins, and 3) sepsis. Feasibility of TPN depends upon close adherence to an optimum protocol for insertion and maintenance, whereby associated complications can be minimized. This report describes the etiology, incidence, recognition, and treatment of catheter complications in TPN and the contribution a trained intravenous nurse can make to a hospital's success with TPN.

摘要:
Imagery is used as an adjunct to I.V. therapy for patients who must undergo more than one attempt for successful initiation of venipuncture. I.V. nurses are encouraged to teach this technique to their patients as a method to enable patient coping and to foster patient participation in care through a learned self-regulatory technique. Patient responses to venipuncture are described as a cycle that can be interrupted with the use of relaxation breathing and guided imagery. The patient learns a useful technique for the initiation of venipuncture and a coping mechanism available as a self-regulatory measure. The I.V. nurse enjoys the success of promoting patient participation and venipuncture with a minimum of discomfort.

Health care professionals nationwide are becoming increasingly aware of the high cost of health care as well as the trend to get the patient out of the hospital and back to his/her home environment. One major stumbling block to this concept has historically been the Medicare guidelines, which specifically address the patient's “homebound” status as a prerequisite for home care. Although in recent years, the focus has been on discharge planning well in advance of the actual discharge date, only recently has this emphasis included the care of the I.V. therapy / nutritional support patient. This account, therefore, addresses the actual patient care of this particular patient population as well as the author's role with VNA in developing an I.V. Certification Program.

摘要:
Infiltration of cytotoxic medications into the subcutaneous tissue can be a devastating sideeffect of chemotherapy treatment. Since nurses are usually the ones giving antitumor agents, it is imperative that specific guidelines are followed in order to allow for safe administration of these agents. Although the nurse has exercised the utmost care during therapy, extravasation of a chemotherapeutic drug can occur. In addition, certain patients will have a greater risk for the potential extravasation of chemotherapeutics. Few recommendations for treating extravasations have been scientifically established; however, with the use of specific guidelines progression of toxicity can be prevented.

The currently available parenteral penicillinase-resistant penicillins include oxacillin sodium, methicillin sodium, and nafcillin sodium. In recent years, of these three antibiotics, practitioners have increased their utilization of nafcillin sodium. This has most likely been caused by evidence1 that suggests methicillin sodium is more frequently associated with nephropathy than is nafcillin sodium. Probably as a result of increased usage of nafcillin sodium, reports are now appearing that describe extravasation necrosis in conjunction with parenteral administration of this drug.2'3 All reports to date have been in young infants aged four months or younger. We have now observed nafcillin necrosis occurring in an adult.
