AbstractProthrombin complex concentrates prepared for clinical use are reviewed with emphasis on frequently observed adverse reactions such as viral hepatitis and thromboembolic complications. Preparation procedures and quality control are described briefly, and the use of these concentrates in the treatment of hemophilia A patients who develop factor VIII inhibitors is also described. The causes of and methods for the detection of potential thrombogenicity are discussed and suggestions which may result in a safer product are made.

AbstractRed cells stored under blood bank conditions normally show less than 1% spontaneous in vitro hemolysis even after 5 weeks; larger hemolysis may be found if the cells are suspended and stored in a saline‐adenine‐glucose (SAG) solution with very little trapped plasma. Delay of the addition of the suspension medium, return of 25 ml plasma after a maximal plasma harvest, addition of mannitol 10–30 mmol.1‐1to the suspension medium were alternative and effective ways of keeping the spontaneous lysis within normal limits. Mechanical traumatization (centrifugation or shaking) caused considerably more damage to the red cells when these were highly concentrated than when they were diluted. AbstractAliquots of 213 specimens were assayed for C3, C4 and CH50. A graphical presentation of the data was obtained by a block diagram. This showed a generally flat plane with good agreement to quoted normal ranges.Only 2 of 68 specimens with C3 and C4 normal have a low CH50. If either the C3 or C4 is low then the CH50 may be normal or even high, but if both fractions are low then so is the CH50.A low CH50 can occur with both C3 and C4 in the normal range or even higher. This implicates other complement factors as being deficient with a sufficiently high occurrence rate to require that the CH50 be kept as a routine laboratory procedure.

AbstractMeasurements have been made of the absolute T‐cell loss experienced by normal donors undergoing three methods of pheresis used for the preparation of platelets and/or granulocytes for transfusion. Filtration leukapheresis was associated with a mean loss per donation of 1.7×108T cells. The technique of intermittent flow centrifugation platelet pheresis resulted in a mean loss per donation of 3 × 109T cells. When this latter technique was utilized for leukapheresis, with hydroxyethyl starch and prednisone taken 12 h before the donation, the mean loss per donation was 3.9×109T cells. When an intravenous dose of dexamethasone at the commencement of the procedure was used instead of prednisone for leukapheresis by intermittent flow centrifugation, the mean cell loss per donation was 2.2×109T cells. We have examined the immediate effects of these procedures on circulating T lymphocytes and their responsiveness to mitogens. The only change of significance was an increased responsiveness to phytohemagglutinin after leukapheresis of the lymphocytes of those donors who had received prednisone. Replacement of T cells appears to be inefficient in adult life and thus functionally important numbers of T lymphocytes may be removed by repeated pheresis procedures. AbstractVasculitis is an uncommon, extremely debilitating complication of severe, chronic rheumatoid arthritis. High levels of circulating immune complexes along with vascular deposition of immunoglobulins and complement indicate an immune complex mediated disease. No pharmacologic therapy has been completely effective and the incidence of mortality is exceptionally high. This study shows clinical improvement with combination of plasma exchange and cytoxan therapy. We report an association in the conspicuous decline of immune complexes with objective clinical remission in 2 patients who underwent combination therapy. This adjunctive combination therapeutic modality is recommended in the treatment of rheumatoid vasculitis.

AbstractPlatelet antibodies were looked for in 47 patients with autoimmune thrombocytopenia using a modification of the enzyme‐linked assay previously described. Surface‐bound antibodies measured as increased platelet‐associated IgG were found in 32 (68%) of the patients. After incubation in test sera, the platelet‐associated IgG of normal donor platelets was significantly increased in 27 of the 47 patients (57%), thus demonstrating the presence of platelet antibodies free in their sera. 6 patients had antibodies only in the serum without any elevation of their platelet‐associated IgG. When both tests are evaluated together no antibody was detected by either the direct or the indirect test in 9 of the 47 patients (19%) studied. AbstractFluorochrome‐labelled bacteria were tested for their rosette‐forming properties with human lymphocytes in suspension. Acridine orange stained human buffy coat cells or isolated mononuclear cells are rosetted with tetramethyl‐rhodamine‐isothiocyanate (TRITC)‐labelled bacterial strains alone (mono‐bacterial rosetting test) or simultaneously with a TRITC‐labelled strain and a mutant or taxonomically different strain, labelled with fluoresceinisothiocyanate (double bacterial rosetting test [D‐BRT]). Suspensions are centrifuged, washed, finally counterstained with ethidium bromide and examined by fluorescence microscopy. The bacterial binding properties of B cells may be studied by using anti‐Ig pretreated mononuclear cells and TRITC bacteria (anti‐Ig/BRT).
