AbstractModern techniques of blood collection require health screening of donors by non‐physicians using general guidelines. Health problems of concern to the donor and possible loss to future donation of actually healthy individuals are inherent in this acceptance and deferral process. Of the 108,908 donors presenting at the bloodmobiles of a regional blood center from September 1978 to February 1979, 9,542 were deferred. Of these, 1,203 with possible medical problems other than hepatitis, hypertension, or infectious disease were referred to their physician for further information concerning their suitability as donors. Over 80% had irregular pulses or histories suggestive of potential cardiovascular problems. The purpose of this study was to review the evaluation mechanism. Responses for evaluation were received from only 222 out of 1,203 (18%), and of these all but 29 (13%) were granted full permission to donate. Since only one evaluated individual with pulse or cardiovascular findings of possible significance was under the age of 35 years, this age is a reasonable cutoff for such deferrals. AbstractA commercial plasmin‐treated human immunoglobulin was studied for its antibody spectrum and anticomplementary activity. Except for a decrease in anti‐vaccinia antibody titers in two of three paired samples collected before and after plasmin treatment, there was practically no fall in titers of diphtheria antitoxin and of antibodies to measles, rubella and mumps viruses. Among the three major fractions isolated from the preparation, the plasmin‐resistant 7S IgG fraction showed the highest anticomplementary activity when tested by our routine method described in the ‘Japanese Minimum Requirements’. The Fab fraction showed moderate anticomplementary activity which was thought to be due to the selective consumption of C3. On the contrary, the Fc fraction was not found to be anticomplementary in our test method. AbstractCovalently linked albumin dimers and polymers had plasma half‐lives (t 1/2s) in mice which were not significantly different from that of native albumin. Abstract58 patients were treated by discontinuous flow plasma exchange because of an immune complex (IC) disease. 41 patients who had a high level of circulating IC prior to plasmapheresis showed both clinical and immunochemical evidence of improvement with plasma exchange. Only 2 patients with a high level of IC did not improve after therapy: these patients suffered from multiple sclerosis and idiopathic thrombocytopenic purpura, respectively. Prior to PE therapy the level of circulating IC in 15 patients was within the normal range, when measured according toMancaet al. In this group none of the patients worsened after therapy, whereas 8 patients showed an objective improvement. The positive correlation between IC removal and clinical results suggests that patients with a high level of circulating IC are most likely to benefit from apheretic treatment.

AbstractThe magnitude of the pH change during platelet concentrate storage at 20–24°C in polyvinyl chloride containers is not determined solely by platelet count per cubic millimeter of plasma, since a wide variation in pH was observed with similar platelet concentrations. In modified platelet concentrates having lost through centrifugation 3–15% of total platelets and 61–92% of residual leukocytes, the pH was maintained at substantially higher levels than in the paired control platelet concentrates. Leukocyte levels do not appear to determine the magnitude of the pH fall. Continuous oxygen ulilization is needed if the pH is to be maintained near 7.0. However, oxygen tension per se is not the factor which influences the extent of pH change. It has been concluded that a specific platelet subpopulation comprising a small proportion of the total platelets in concentrates and having an enhanced capacity to form lactate may be responsible for a major part of the pH reduction which occurs during storage of many platelet concen

AbstractA new method for eluting red cell antibodies using chloroform has been shown to be effective. The method is similar to ether and xylene techniques but can be completed within 10 min after adequate cell washing. Comparison studies using ether, xylene and chloroform showed that antibodies eluted by chloroform yielded equivalent titration scores. Antibodies within the Ss blood group system were easily eluted using chloroform but not using ether. Also, the chloroform method yielded informative eluates when prepared from red cells of patients with warm antibody autoimmune hemolytic anemia, drug‐induced immune hemolytic anemia, hemolytic disease of the newborn caused by ABO or Rh fetal‐maternal incompatibility, or from patients having a positive direct antiglobulin test as a result of alloantibodies stimulated by recent transfusion (‘delayed transfusion reaction’).
