SummaryInformation obtained from animal experiments cannot generally be applied to clinical situations, but such obvious differences as the difference between the healing of drained and undrained anastomoses may be applicable. All 15 of the control dogs recovered, and their colonic anastomoses had only filmy adhesions and no stricture formation. In contrast to this, of those with Penrose drains, nine of 20 died of anastomotic disruption and peritonitis, and the rest had extensive adhesions and varying degrees of stricture formation.The use of drains cannot be condemned categorically because in gross and microscopic appearances the anastomoses in seven dogs with Teflon tape drains were not much different from those of the control animals.

Summary and ConclusionsThe case records of 369 patients (of an original 1,786) who lived more than ten years following initial treatment for cancer of the rectum have been analyzed: 265 patients were alive and well after 15 years, 147 after 20 years, and 47 after 25 years. Very few died of recurrent rectal cancer beyond the first decade following initial treatment.Metachronous polyps developed in only 6.3 per cent of the 309 patients who could be followed for this long period. However, subsequent new colonic cancers developed in 17 (5.4 per cent) of these patients; 13 developed the second cancers during the second decade after the original treatment.Noncolonic cancers also appeared in 9.5 per cent of these 309 patients, an incidence far greater than that in the general population.Nine per cent of the 309 patients required surgical repair of paracolostomy hernias.The overall survival rates for 551 patients at the 5‐, 10,‐ 15‐, and 20‐year levels, tabulated from the stand‐point of Dukes' classification, are presented, and reconfirm a mortality attrition due more to other causes than to recurrent rectal cancer.

SummaryThe three case reports which have been presented show the seriousness of rectal injuries (and ensuing complications) that can be caused by administration of barium enemas. I am sure that these injuries can be prevented if the enemas are administered carefully. However, when such trauma does occur, complications can be reduced by early recognition and treatment.

SummaryA case in which a crushing injury to the pelvis resulted in complete loss of the anorectum, together with the levatores ani, is reported. Management of the patient, particularly the use of a reversed loop of ileum as a replacement for the anal sphincter and rectum, is described. Reports in the literature of crushing injuries to the pelvis which caused avulsion of the rectum are noted, and an explanation of the mechanism likely to be involved in avulsion and expulsion of the rectum is given.
