Summary and ConclusionsRegional colitis may involve any segment of the colon with or without concomitant involvement of the small bowel. It is an entity, separate and distinct from ulcerative colitis. Usually the correct diagnosis can be established by clinical, radiologic and histologic examination.Careful rectal biopsy, or microscopic examination of tissue removed from anal and perianal lesions, may reveal granulomas or sarcoid reactions typical of the disease.Because most patients with regional colitis limited to thecolon onlydo very well after surgery, prompt resection and anastomosis is advocated.In regional colitis, the rectum should be preserved if possible, even if a temporary ileostomy is required in order to treat severe perianal lesions. There is little danger that the rectum will become involved by carcinoma or that systemic complications such as arthritis, uveitis, pyoderma and hepatitis will occur.

ConclusionsThe bacterial species that are most abundant in the gastrointestinal tract under normal conditions are anaerobic and have exacting growth requirements. As they display only a narrow range of biochemical activities, it is probable that the chemical transformation of metabolites in the gastrointestinal tract is less pronounced when these organisms predominate than when others gain the upper hand under pathologic conditions. In other words, the composition of the gastrointestinal flora determines the nature of the bioactive substances that are produced through metabolic transformation of amino acids, bile acids and other metabolites. Such biochemical activities may be as important as the orthodox pathologic lesions caused by pathogens.The microorganisms of the indigenous flora exert morphogenetic effects that are essential for adequate histologic development and for the healthy function of the gastrointestinal tract. The most important species of this flora are not necessarily those which multiply most abundantly within the lumen, but rather those which selectively become associated with the mucous layer in the different parts of the gastrointestinal tract.In brief, the microbial flora affects the histologic structure of the gastrointestinal tract and the kind of substances that are released from it into the general circulation. The different parts of the digestive tract, the various microbial species that they harbor selectively, and the physiologic conditions that govern the interplay between the host and its indigenous flora constitute a highly integrated ecosystem. Interference with any component of this system is likely to disturb its equilibrium and therefore to result in pathologic manifestations.

Summary and ConclusionsOpinions of authorities differ concerning what constitutes a successful result after ileorectal anastomosis. No controlled studies have been undertaken to provide an accurate estimate of the percentatge of patients with ulcerative colitis who can anticipate a successful ileorectal anastomosis. It seems impossible to predict which patients will have successful results and which will not. One‐stage and two‐stage operations seem equally satisfactory. No special postoperative complications occurred in our series. Some patients require constipating drugs after ileorectal anastomosis. As a rule, the rectum does not return to normal after ileorectal anastomosis. Most of our failures have occurred during the first 12 months after operation. Re‐intervention can be difficult. The danger that cancer will involve the rectal stump may not be as great as we have feared.

ConclusionsWe believe that carcinoma of the colon is a multicentric organ disease and that a significant number of curable patients are lost because of the development of synchronous or metachronous tumors. These may progress to an inoperable stage before recognition.This group of patients can be saved by a near‐total colectomy at the time of the initial procedure.More radical resection of the colon did not increase mortality or morbidity rates to a significant degree in our select group of patients. The ease of follow up by sigmoidoscopy has made it feasible to detect and treat second lesions at the earliest possible moment.In those patients whose prognosis is good for a long life expectancy and in whom the local lesion is considered curable, near‐total colectomy appears to be the operation of choice for carcinoma of the colon.
