SummaryThe incidence of multiple primary adenocarcinomas of the colon at Charity Hospital in a 19‐year period was 2.1 per cent. The incidence of all multiple colonic and rectal malignancies was 4.1 per cent. Thirteen patients (0.5 per cent) had multiple metachronous adenocarcinomas, and 34 patients (1.6 per cent) had multiple synchronous adenocarcinomas.The possibility of a second carcinoma makes it imperative for the physician to obtain good diagnostic studies, bariumenema studies, and proctoscopy of every patient preoperatively. The resected specimen should be examined by the pathologist at the time of operation. Postoperatively, an annual physical examination, including proctoscopy and barium‐enema studies, is necessary.

SummaryA method of long‐intestinal‐tube gastrostomy with internal splinting of the small intestine is presented. Indications and advantages are described. The method has been successful in a six‐year experience. Its simplicity, efficiency and freedom from complications are emphasized. Inflation of the bowel by a soft ballon, facilitating dissection in navigating the bowel, is also stressed.

SummaryExperience with the use of ketamine hydrochloride combined with local infiltration anesthesia in 200 anorectal surgical patients is reported, and a method of aborting or preventing unpleasant psychomotor reactions is described. On the basis of this study, ketamine hydrochloride with infiltration of local anesthetic is recommended for use in anorectal surgery.

SummaryMegacolon may be classified as primary or aganglionic, secondary or acquired, and functional or psychogenic. In addition, there are other mechanisms, currently poorly understood, that cause megacolon.Diagnostic studies of the patient with megacolon should include a detailed history, physical examination, rectal and sigmoidoscopic examinations, a rectal biopsy, and (for functional megacolon) psychiatric evaluation. Age of onset, encopresis, and dilatation and elongation of the colon with a narrow rectal or rectosigmoid segment are import details to consider in the differential diagnosis. Biopsy of the rectum will prove the presence or absence of ganglion cells. Careful attention should be given to patients with constipation, lest some of them be unnecessarily relegated to lifetimes of avoidable embarrassment and suffering.We have described the cases of four patients with megacolon who had reached their teens or even young adulthood before being treated. The Swenson pull‐through procedure produced excellent results in each case.

SummaryThree patients with familial polyposis in childhood have been presented. Two siblings were treated via total colectomy and ileoproctostomy with preservation of the ileocecal valve. Residual rectal polyps have remitted spontaneously. Eleven cases of carcinoma complicating familial polyposis in children have been reviewed. Carcinoma may cause death prior to the age of 16 years. Total colectomy and ileoproctostomy is the preferred treatment in childhood. The adjustment to an ileoproctostomy involves minimal postoperative morbidity and few psychological problems. The hazard of carcinoma in the retained rectum is minimal in children.

SummaryThe literature on this subject has been reviewed. A personal series of 18 patients more than 50 years of age, treated by major operations for ulcerative disease of the large bowel, has been presented, and the deaths occurring in this group have been discussed. Attention has been called previously to the insidious and lethal nature of this disease. These observations have been corroborated by this study. The importance of early diagnosis and aggressive surgical management has been emphasized.
