SummaryDuring a 201/2‐month period, 115 surgical patients with intestinal diseases were evaluated for possible systemic infection. In 101 patients infection was suspected; antimicrobial therapy was administered promptly on the basis of a presumptive diagnosis after appropriate smears and cultures were obtained, but before the causal organisms were definitively identified or theirin‐vitrosusceptibility was determined. Of the 101 patients, serious infection was proved in 79; septicemia occurred in 25 per cent. Septic shock developed in 15 patients; only five died. The low mortality was attributed to prompt diagnosis and treatment. Further reduction in the incidence and mortality of septic shock may result from utilization of appropriate preventive measures.

Summary and ConclusionsThe cases of a series of patients who underwent elective resection of the left colon and rectosigmoid in a 31‐year period are reviewed. Results in those in whom subhepatic transverse colostomies were established are compared with results in those not so treated. The authors believe that diverting colostomies are effected much too often. They are needed, however, for treatment of complete colonic obstruction and when inflammatory processes are so severe that they are life‐threatening.

SummaryIn two groups of 20 cats each, the healing processes in resected and anastomosed bowel after use of a stainless steel surgical knife and an electric knife were studied. The animals were sacrificed after 24 and 48 hours, and four, six and 11 days. The anastomoses were resected and specimens were examined. Histologic study of specimens of tissue from Group I (stainless steel surgical knife) on the fourth and sixth postoperative days revealed proliferation of fibroblasts, granulation tissue, epithelial healing, and fusion of the opposed aspects of the muscularis. The healing process in Group II (electric knife) was characterized by necrosis of tissue, exudate of fibrin and leukocytes, and incomplete fusion of the opposed muscularis. Histologic studies showed a lag in total tissue repair of two days in Group II. These results suggest that the stainless steel knife is preferable for colonic surgery.

SummaryWe have chosen four cases illustrating pitfalls and difficulties in the diagnosis of amebic colitis and the disastrous results of misdiagnosis. In many instances the correct diagnosis is not reached because of insufficient awareness of the possibility of the disease on the part of the physician, or because of his reliance on examination of the stool alone to exclude amebiasis. In other cases, as in amebic appendicitis or amebic typhlitis, the clinical picture may be identical to that of bacterial appendicitis.The most reliable test for diagnosis of the disease is the examination of material from ulcer scrapings or biopsy.Our paper is presented as a reminder that amebic infection and amebic colitis still exist and it is essential to identify them, not only for academic reasons, but mainly to ensure that the patient receives the correct treatment, thereby preventing suffering and complications that may, at times, prove fatal.
