Summary and ConclusionsA brief history of lymphogranuloma with special reference to rectal involvement is given. Experience in Jamaica in the treatment of 144 cases of rectal lymphogranuloma is reported. In cases of proctocolitis patients can be relieved of rectal stricture only by its removal but symptoms may be relieved by more conservative treatment. Excision operations should be reserved for the severe cases. The prognosis of proctectomy with preservation of sphincter function should be guarded. The abdomino‐end‐anal operation of Dimitriu and Gregoresco has given promising results.

SummaryResults are reported on the effect of striated muscle fiber relaxants, in decreasing reflex muscle contracture of the anal sphincter muscles caused by the first postoperative evacuation of the rectum, in patients having undergone hemorrhoidectomy or combined fissurectomy and hemorrhoidectomy. Twenty patients were given tubocurarine chloride injections in a repository vehicle, while three groups of 15 patients each were treated with meprobamate, methocarbamol and chlormezanone, respectively.A comparative study was made of another group of 20 patients who had undergone the same surgical treatment and who received the same care, excepting administration of muscle relaxants.Patients treated with tubocurarine or meprobamate reacted favorably and pain was reduced. The conclusion was reached that both drugs are valuable in lessening pain intensity after anorectal surgery.Methocarbamol and chlormezanone did not render favorable results according to the observations made in this study.

SummaryExperience has shown that by clearing the bowel and introducing carbon dioxide, fulguration of polyps can be performed easily and with no danger of explosion. The apparatus described is simple, inexpensive and makes possible an efficient method of introducing carbon dioxide.

SummaryRecords of a series of 28 patients with sigmoidal fistulas have been reviewed. Our discussion of the incidence and pathogenesis of sigmoidal fistulas is based on a review of the literature and a study of a collected series of cases. The value of various diagnostic features (symptomatology, physical examination, sigmoidoscopy, roentgenography, urologic studies, Papanicolau staining methods, direct biopsy and needle biopsy) have been discussed. The historical background of the surgical management of sigmoidal fistulas has been presented. The results of treatment in the present series have been summarized. A general discussion of the surgical management of sigmoidal fistulas is based on personal observation and a study of a collected series of cases.

Summary and ConclusionsDelayed hemorrhage after anorectal surgery is a problem which concerns most of us, as responses to a questionnaire have shown; some proctologists reported no such complication in their experience. Etiology is discussed and technical features have been described which should overcome the difficulty.A balloon‐ball rectal tube, a double balloon catheter and a univalve anal speculum have been presented and the method of using them has been described.Most cases of delayed postoperative bleeding can be handled without operative intervention. In our hands, no more blood was given by transfusion when nonoperative procedures were employed than when surgical intervention was required.

SummaryThe presence of excessive hair on the glabella constitutes a significant sign which is helpful in detecting individuals with pilonidal disease. The records of 5,477 individuals have been reviewed and 46.6 per cent had this sign. One of five patients with the positive sign had an inflamed pilonidal cyst and 93.3 per cent of all patients who had pilonidal disease had the positive sign. When this sign is observed at the time of enlistment of military personnel, it is likely that a careful examination will reveal that previously undetected pilonidal disease will be discovered. As a result there should be a great saving in time and expense in the management of these cases.
