SummaryInfectious syphilis is being reported in increasing numbers and the prevalence of homosexuality in males is one reason for its increase. An improved awareness of the possibility that syphilis may be the cause of anal infections is necessary. Eleven cases of infectious syphilis of the anal canal are presented.

Summary and ConclusionsA series of 118 patients was studied following hemorrhoidectomy to evaluate the effect of topically applied hydrocortisone on wound healing. Sixty of the patients received hydrocortisone‐containing suppositories, while 58 were given inert suppositories and served as controls. Wound healing was evaluated, and the patients were classified on the basis of excellence of healing. The treated patients and controls were then identified in accordance with the double‐blind method, and the results were compared. The results were subjected to statistical analysis. On the basis of this study it appears that administration of hydrocortisone‐containing supposiories during the period of convalescence following hemorrhoidectomy has no significant effect upon wound healing.

ConclusionAn anatomic study has shown that anal glands penetrated the internal sphincter in half of a series of 44 patients.In an unselected series of 30 consecutive patients with anal fistula, the histologic appearance in 21 strongly suggested that the cause of the disease was infection in anal glands.An operative technic has been described which was used in 38 consecutive cases. The aim of this method is to eradicate the source of the disease which is believed to lie between the internal and external sphincters,without dividing the external sphincter muscle. This can be quite a difficult technical procedure in some cases and, at present, it is not generally recommended for low fistulas; it is of great value, however, in dealing with high fistulas as it spares the mutilation of extensive sphincter division.

SummaryA review has been presented of the historical background and pathogenesis of acute fulminating ulcerative colitis with distention of the colon. A personal series of ten cases has been summarized and symptomatology, physical findings, diagnosis and treatment have been discussed. Opinions have been expressed regarding problems involved in the management of this severe complication and recommendations have been made concerning the optimum surg cal procedure.

Summary and ConclusionsHighly malignant and distinctive transitional cell tumors of the anorectal area can arise from inconstant and persisting embryologic entodermal cloacal vestiges situated just above the dentate line. These nonkeratinizing lesions are quite similar histologically to the transitional cell tumors found in the cloacogenic portion of the lower genitourinary tract.Microscopic features of these junctional tumors vary somewhat, depending on the site of origin from the complex cloacogenic zone and the mixed epithelial components involved. The cellular patterns of the transitional cell anorectal tumors differ distinctly from the more common rectal adenocarcinoma, the keratinizing squamous cell tumor of the anal canal and the very rare basal cell perianal skin lesion.Most of the cloacogenic upper anal canal and lower rectal junctional lesions occur in women. Many of the reported transitional cell anorectal lesions have metastasized rapidly. The prognosis appears to be graver than that of more common rectal adenocarcinoma and the keratinizing squamous cell anal lesion. Early diagnosis and prompt radical surgical removal seem to offer the best hope for survival.

SummaryAn appeal is made for a standardized method of intestinal antisepsis.It is urged that investigators, who would evaluate the effectiveness of intestinal antisepsis, emphasize that the specific procedure studied yielded either satisfactory or unsatisfactory results. The procedures that I have presented have never been proved inadequate by scientific demonstration, whereas there is ample evidence, both clinical and experimental, to prove their efficacy. The problem of malignant cell transplantation into the anastomotic line is discussed.Intraperitoneal use of neomycin solution is discussed, and it is again urged that both the surgeon and the anesthesiologist acquaint themselves with physiologic facts which should convince them that they should cease denying patients the benefits of this life‐saving procedure.
