ConclusionsProgress in the over‐all control of rectal cancer can be accomplished in the following manner: by achievement of the highest possible resectability rate; by well‐designed and executed surgical procedures; by employment of adjuncts available now and continuance of vigorous and imaginative efforts to find more effective agents; by ceaseless efforts to detect and remove polyps; by earlier diagnosis accomplished by full employment of means currently available and, hopefully, new screening methods; by clinical research in the field of immunology; by cooperative efforts to pool all types of data relevant to patients with cancer of the rectum; by long‐term follow‐up study of large populations subjected to examination for cancer of the rectum, and by epidemiologic studies of the population with regard to cancer of the rectum.It is of equal importance to discourage regressive measures which threaten and even obstruct effective types of treatment which are available.

ConclusionsIt can be concluded that polyposis of the colon is a multifaceted disease. The mendelian character of the disease cannot be denied in patients belonging to the same family, especially when they are less than 40 years of age. The occurrence of polyposis in patients more than 40 years of age, or in those in whom no familial antecedent can be traced, is more difficult to comprehend. Probably the occurrence of this type of polyposis is on the same basis as that of carcinoma of the colon and, if this is true, it might be termed primary multiple polyposis of the colon.

SummaryCryotherapy was used for palliation of symptoms in seven patients with inoperable rectal cancer and in one with perineal recurrence of cancer after abdominoperineal resection. Bleeding was controlled, obstruction was relieved and colostomy was not required. These benefits were related to reduction of tumor bulk. It appears that cryotherapy can compete successfully with irradiation and electrocoagulation in the management of selected cases of inoperable rectal cancer. Ease of administration, relief of symptoms, lessened risk, and avoidance of colostomy are advantages which indicate the general suitability of cryotherapy for the patient with inoperable cancer who cannot tolerate major operations.

SummaryDespite the accessibility of the anorectum, 20 per cent of hemorrhoidectomies fail to attain satisfactory anatomic and functional end results.Late disabling complications of hemorrhoidectomy include anal stenosis, anal fissure,re‐formedhemorrhoids, prolapse of rectal mucosa, excessive tags and, occasionally, suture granulomas and perianal epidural cysts.The causes, prevention and management of such lesions have been discussed.

SummaryOne hundred and eight patients underwent surgery for complications of diverticular disease of the right colon at The Queen's, St. Francis, and Kuakini Hospitals during the ten‐year period ending December 31, 1965. Ninety‐two per cent were non‐Caucasian Orientals.Surgery was required for acute diverticulitis in 58 patients, chronic diverticular disease in 43, hemorrhage in six, and obstruction in one.Colectomy was performed as the primary procedure in 67 patients, diverticular surgery was employed in 31, and 10 patients underwent exploration. Three of the last group underwent colectomy subsequently. There was no postoperative mortality.In Hawaii, diverticular disease of the right colon appears to be a distinct clinical entity. It occurs most commonly in the 40‐year‐old non‐Caucasian Oriental, Usually the diverticula are multiple and colectomy provides satisfactory results.

SummaryA case of Hodgkin's granuloma of the colon is presented and the importance of distinguishing it from Hodgkin's sarcoma is emphasized. The types of therapy which are now approved include surgery and irradiation, or chemotherapy, or both. It is hoped that meaningful conclusions concerning the results of present therapy will improve as additional cases of Hodgkin's disease of the colon are encountered.

SummaryOur experience with 118 patients who underwent 123 consecutive liver scans has been reviewed. The accuracy of preoperative liver scanning, as well as other liver function tests, was assessed subsequently at laparotomy, at autopsy or by biopsy. Liver scanning predicted liver metastasis correctly in 72.0 per cent of cases, and alkaline phosphatase was elevated in 50.0 per cent of those patients who had metastatic liver disease.
